HCA340 WEEK 4 ASSIGNMENT

S.W.O.T. (Strengths, Weaknesses, Opportunities,
Threats)

For this assignment, using the situation given below,
apply the S.W.O.T. Situational Planning Strategy which was described in Chapters
4 and 7 of your course text:

Situation: You are the C.E.O. of Community South Medical
Center, a large, urban for-profit healthcare facility. This institution has
comprehensive health services including acute care, residential care,
independent living, in-home nursing, hospice, neonatal, and pediatric services,
advanced cardiac services, a major trauma center, a center of excellence for
pulmonary services, and a neurosurgery center.

Community South Medical Center has a positive bottom line and is striving to
enhance health services for the population it serves. The patient care mix has
shifted in the last three years and its reimbursement source is currently: 25%
Medicaid (up 20%); 35% Medicare (down 12 %); 25% employer sponsored health
insurance (unchanged), 8% managed care (down 20 percent), 4% private pay
(unchanged) and 4% no-pay (up 25%).

The Medical Center is in an older urban area. Businesses with well-paying
jobs have gradually been replaced by smaller shops and other small businesses.
Overall, the community is trending towards a predominantly elderly population.

The Medical Center has an excellent reputation and has been recognized in the
past by receiving a Baldrige Center of Excellence evaluation and a The Joint
Commission (T.J.C.) approval for their quality of services; however, recent
self-inspections indicated a slight decline in compliance with T.J.C.
standards. Patient satisfaction survey results have an overall mean of 95
percent. The medical staff strongly supports new program development and there
is an abundant supply of physicians.

Currently, the major issues confronting the Community South Medical Center
are: an identified shortage of clinical staff including registered nurses,
respiratory therapists, and medical technologists; non-interfacing information
technology systems; antiquated facilities and infrastructure; decline in T.J.C.
compliance on internal audits; and a shift to its financial mix.

Action: As the C.E.O., it is time to embark on the annual
updating of the organization’s strategic plan. The last major revision to the
strategic direction was completed three years ago.

1. After reviewing chapters 4 -6 please answer the following questions. Please no intro and no conclusion. Each question should be at least 220 words. PLEASE UNDERLINE BOLD EACH QUESTION INSIDE PAPER. I will attach the chapters to this order. Q1, in 100 words answer the following question. ? In 220 words answer q1. Many companies stress the use of formal “root cause analysis” to determine why accidents occur and to help identify interventions that will preclude event reoccurrence. Others feel that the root cause techniques are complex, hard to use, and often find management is the cause of every event. Do you think a company should invest in a formal root cause analysis program, and invest in training of lead auditors in the technique? In 220 words answer q2. 1. As the lead plant safety professional, you are asked to chair an accident analysis committee reviewing the following incident:
A worker changing light bulbs in a high bay work area of a unionized shop fell to his death from the basket of the man-lift crane (example: http://www.alibaba.com/productshowimg/in100677380-100435769-0/man_lift_crane.html) he was using. Three co-workers but no supervisors were present. With the crane extended, the worker had climbed up the side rails of the work platform and fell over. He was not using a safety line. Only a week before, another worker had narrowly escaped injury when he slipped using the same crane but caught himself before he fell.
a. Who would you want as team members on the investigating committee, and why?
b. List several questions that you will want your committee to answer.
c. What groups of employees/supervisors/others will you want to interview during the investigation, and why?
In 220 words answer Q3 A proposed engineering control is expected to cut the accident rate by 40 percent for a given process that was recently cited as being out of compliance by an OSHA inspector. Auditors have estimated cost data for accidents with this process as follows (average cost per accident):

Direct Cost Hidden Cost Freq/Yr

First aid cases 1000 4000 6
Med. treatment cases 6000 9000 1.5
Lost time cases 3000 50000 0.5
No-injury cases none 2000 5.0

a. Calculate the total benefit expected for this proposed engineering control. Show your calculations.

b. If the proposed engineering control will cost $40,000, does the cost-benefit analysis support funding it? Why or why not?

c. What other reasons might support funding the project?

What will you discover as you investigate changes in your community and the broader health care landscape?

What will you discover as you investigate changes in your community and the broader health care landscape?

Imagine that you are an administrator in a health services organization, and the CEO of your organization asks you to assess and report on developments that are likely to affect the organization’s success. What will you discover as you investigate changes in your community and the broader health care landscape?

In preparation for this Assignment, review your completed Week 3 Assignment, including any feedback you received from your Instructor. Select one organization from that Assignment on which to focus for this Assignment (i.e., imagine that this organization is the one in which you work).

Use the Learning Resources provided in this week, as well as previous weeks, and locate additional sources through credible websites to conduct your research. Use the following questions to guide your investigation:

What are the prevalent health concerns in this community?
What are the key issues related to cost, quality, and access that affect the organization and the people this organization serves?
Which organizations in this area are business competitors and/or which organizations use highly effective practices or strategies that this organization may learn from or adopt?
How will demographics and health reform likely impact the organization, as well as the broader community, in the next 5 years?

Describe the information and knowledge needs of nurses in all practice domains and settings, electronic health records (EHRs), and patient-care information technology (IT).

6. Describe the information and knowledge needs of nurses in all practice domains and settings, electronic health records (EHRs), and patient-care information technology (IT). (PO 4)

Key Concepts

Explore informatics competencies and health information technology (HIT) that supports patient care and documentation.
Evaluate HIT capability to promote safety.
Develop a view of different types of patient-care technologies.
Examine technology options to support all facets of nursing and patient care.
In this week’s discussion, we will be examining knowledge generated through informatics.

The HealthIT Hot Topic of the Week Assignment, is due at the end of this week. You should contact the Chamberlain writing resources now if you require assistance.

This assignment will be going through Turnitin.

About the HealthIT Hot Topic of the week –

Go to the site: http://www.fiercehealthcare.com/it

Choose one of the topics you are interested in: Analytics, EHR, Mobile, Privacy & Security, Regulatory. Choice one of the articles on that page (or one of the following pages) that may impact your practice. Do not choose a topic from the Healthcare or Payer section. Be sure you use an article from the Fierce Health IT website as the foundation for your paper. Make sure it is an informatics topic. If you are unsure, email me with your topic as well as the article that you plan to use as the starting point of the paper.

In a professionally developed paper, discuss the rationale for choosing the topic, how it will impact practice in a positive or negative manner, citing pros and cons. Include a discussion of how informatics skills and knowledge were used in the process of developing the assignment. In the conclusion, provide recommendations for the future.

Some key points

Be sure to choose a current issue. Discussing the use of EHRs in general is not an appropriate topic. But use of patient portals, or needing EHRs that are more patient centered could be. If in question about your choice of topic, email me. The topic must relate to informatics.
You need to have at least three scholarly resources (refer to the document “What is a Scholarly Source?”) outside of the course resources. Be sure to include the article that you choose as one of your references (and identify it in your paper). FYI – most of the time the article from FierceHealthIT is a newsbrief and therefore not a scholarly article. That means, you will need three additional sources.
The main parts of this paper are:
o Introduction

o Why did you choose this topic – include the FierceHealthcare IT article in the section

o How does it impact your practice in both a positive and negative way, citing pros and cons

o How you used informatic skills and knowledge to complete this paper (word processing, DIKW, etc.). This is NOT how informatic skills apply to this topic.

o Conclusion which includes recommendations for the future and insights you gained in completing this assignment

What terms will you use in order to make sure that your search is wide enough to obtain required information but narrow enough to keep it focused? How will you narrow your search if needed?

Purpose: To identify a problem or concern that nursing can change and develop a PICOT question to guide the change project.
Directions: Use the form below to complete the Week 3 Assignment PICOT Evidence Worksheet. This includes filling in the table with information about your research question and your PICOT elements and the second part is filling in the Evidence Worksheet.
Step 1: Select the key PICO terms for searching the evidence. Clearly define your PICOT question. List each element P (problem, population, or problem), I (intervention), C (Comparison with other treatment/current practice), and O (Desired outcome), T (Time Frame). Is the potential solution something for which you (as nurse or student) can find a solution through evidence research? Look in your book for guidelines to developing your PICOT question and also read the required articles.
Step 2: Identify the problem. What have you noticed in your work or school environment that isn’t achieving the desired patient or learning outcomes? What needs to change in nursing, what can you change with the support of evidence in the literature? Describe the problem or practice issue that you want to research. What is your practice area; clinical, education, or administration? (This is NOT where you will list your PICOT question)
Step 3: How was the practice issues identified? How did you come to know this was a problem in your clinical practice? Review the listed concerns and check all that apply.
Step 4: Identifies and documents four sources of evidence. What evidence must be gathered? What sources of reliable information will be helpful for your particular question? Describes rationale for all checked types of evidence
Step 5: What terms will you use in order to make sure that your search is wide enough to obtain required information but narrow enough to keep it focused? How will you narrow your search if needed?

Discuss Critiquing Quantitative, Qualitative, or Mixed Methods Studies on articles related to Educating the underserved diabetic patient in the primary care setting.

locate two articles in scholarly journals that deal with your portfolio topic: 1) Select one article that utilizes a quantitative research design and 2) select a second article that utilizes either a qualitative OR a mixed methods design. These need to be single studies not systematic or integrative reviews (including meta-analysis and metasynthesis). You may use research articles from your reference list. If you cannot find these two types of research on your portfolio topic, you may choose another topic.•Locate the following documents in this week’s Learning Resources to access the appropriate templates, which will guide your critique of each article:•Critique Template for a Qualitative Study•Critique Template for a Quantitative Study•Critique Template for a Mixed-Methods Study•Consider the fields in the templates as you review the information in each article. Begin to draft a paper in which you analyze the two research approaches as indicated below. Reflect on the overall value of both quantitative and qualitative research. If someone were to say to you, “Qualitative research is not real science,” how would you respond? To complete this Assignment:•Complete the two critiques using the appropriate templates.•Write a 2- to 3-page paper that addresses the following:•Contrast the types of information that you gained from examining the two different research approaches in the articles that you selected.•Describe the general advantages and disadvantages of the two research approaches featured in the articles. Use examples from the articles for support.•Formulate a response to the claim that qualitative research is not real science. Highlight the general insights that both quantitative and qualitative studies can provide to researchers. Support your response with references to the Learning Resources and other credible sources.•As you complete this Assignment, remember to:•Submit your paper to Grammarly and SafeAssign through the Walden Writing Center. Based on the Grammarly and SafeAssign reports, revise your paper as necessary.•Reminder: The School of Nursing requires that all papers submitted include a title page, introduction, summary, and references. The School of Nursing Sample Paper provided at the Walden Writing Center provides an example of those required elements (available from the Walden University website found in this week’s Learning Resources). All papers submitted must use this formatting.•Combine all three parts of this assignment into one Word document including both critique templates and the narrative with your references. Submit this combined document.
