Step Two

For all five problems the client expresses, write a problem statement for each that describes her concerns. Include any preferences you hear the client indicate and draft an initial goal for each problem statement. The problem statements and draft goals should be listed in the provided template.

Step Three

Review the problem statements and initial goals. Then, combine the problem statements and rewrite initial goals to develop more effective and efficient goal statements.

Step Four

Using your new goal statements and the template provided, create a plan of services for the client to include

  • at least three goals
  • the objectives needed to achieve each goal
  • deadlines for achieving the objectives
  • the responsible party (or parties) for each objective
