Course Project: Part 3—Translating Evidence Into Practice A. In Part 3 of the Course Project, you consider how the evidence you gathered during Part 2 can be translated into nursing practice. Now that you have located available research on your PICOT question, you will examine what the research indicates about nursing practices. Connecting research evidence and findings to actual decisions and tasks that nurses complete in their daily practice is essentially what evidence-based practice is all about. This final component of the Course Project asks you to translate the evidence and data from your literature review into authentic practices that can be adopted to improve health care outcomes. In addition, you will also consider possible methods and strategies for disseminating evidence-based practices to your colleagues and to the broader health care field.
To prepare:
1. Consider Parts 1 and 2 of your Course Project. How does the research address your PICOT question? 2. With your PICOT question in mind, identify at least one nursing practice that is supported by the evidence in two or more of the articles from your literature review. Consider what the evidence indicates about how this practice contributes to better outcomes. 3. Explore possible consequences of failing to adopt the evidence-based practice that you identified. 4. Consider how you would disseminate information about this evidence-based practice throughout your organization or practice setting. How would you communicate the importance of the practice?
In a 3- to 4-page paper:
1. Restate your PICOT question and its significance to nursing practice. 2. Summarize the findings from the articles you selected for your literature review. Describe at least one nursing practice that is supported by the evidence in the articles. Justify your response with specific references to at least 2 of the articles. 3. Explain how the evidence-based practice that you identified contributes to better outcomes. In addition, identify potential negative outcomes that could result from failing to use the evidence-based practice. 4. Outline the strategy for disseminating the evidence-based practice that you identified throughout your practice setting. Explain how you would communicate the importance of the practice to your colleagues. Describe how you would move from disseminating the information to implementing the evidence-based practice within your organization. How would you address concerns and opposition to the change in practice?
B. This part of the Course Project should be combined with the other two components of the Course Project ( project 1 & 2) and turned in as your Portfolio Assignment for this course.

Your home health agency has received an order from a local hospital to evaluate and treat an elderly woman being discharged from its medical surgical unit.
Millie Gardner, an 83-year-old female patient, is being discharged home today to the care of her husband Fred (87 years old) following a 9-day hospitalization for pneumonia, dehydration, and failure to thrive. She has a history of hypertension (HTN), Type II Diabetes, and cerebral vascular accident (CVA) with left-sided weakness. Patient is alert and oriented but does have periods of forgetfulness during the overnight hours. Patient has intermittent incontinence of bowel and bladder and requires assistance with all activities of daily living (ADLs).

Discussion 3
Randy Adams is a 38-year-old male patient of Dr. Joseph Reynolds who was admitted yesterday morning for 24-hour observation for mild concussion following a motor vehicle accident. Randy lost consciousness during the accident and was very confused when he arrived in the ER after EMS transport. He is an Iraq war veteran and he seemed to think after the accident that this all happened in Iraq. Dr. Reynolds is concerned that Randy has some residual problems from a couple of explosive incidents that occurred while he was in Iraq. The physician is unsure whether Randy’s current symptoms are from the car accident or from prior injuries so he has referred him for consultations to both a neurologist and to a behavioral health specialist.

Esther Jackson is a 56-year-old black female who is 1-day post-op following a left radical mastectomy. During morning rounds, the off-going nurse shares with you during bedside report that the patient has been experiencing increased discomfort in her back throughout the night and has required frequent help with repositioning. She states that the patient was medicated for pain approximately 2 hours ago but is voicing little relief and states that you might want to mention that to the doctor when he rounds later this morning. With the patient appearing to be in no visible distress, you proceed on to the next patient’s room for report.
Approximately 1 hour later, you return to Ms. Jackson’s room with her morning pills and find her slumped over the bedside stand in tears. The patient states, “I don’t know what is wrong, I don’t feel right. My back hurts and I’m just so tired. What is wrong with me?” The patient refuses to take her medications at this time stating that she is starting to feel sick to her stomach.
Just then the nursing assistant comes into the patient’s room to record Ms. Jackson’s vital signs, you take this opportunity to quickly research the patient’s medication record to determine if she has a medication ordered for nausea. Upon return, the nursing assistant hands you the following vital signs: T 37, R 18, and BP 132/54, but states she couldn’t get the patient’s pulse because “it is all over the place.”
Please address the following questions related to the scenario.
1. What do you suspect is the cause of the patient’s symptoms?

As per the case description of Patrick and following the diagnostic guidelines of DSM-5, the patient can be told to have psychotic disorder due to medical condition with Schizophrenia as predominant symptom (295.90).

Observing the case of Patrick it was found that, Patrick is a homosexual male of 42 years of age living alone since last “six” years. DSM-5 criteria for the clinical condition of Patrick are being listed one after another as follows:
Criteria A: Characteristic Symptoms

1) Since last “two months” Patrick has complaints of having increased anxiety and upset, he also has complaints of sleeplessness. Patrick believes that his colleagues and his boyfriend are behaving crazy with him. As per the case it seems that Patrick has convinced himself and it is hard to change his believes, so there is certain delusion in the discussed case of Patrick.
2) In another context describing his work, Patrick has reported that all his fellow colleagues are jealous of him because he is the favorite employee of his boss. Again he mentions that there are some secret business agents those are helping him by giving valuable advice for his work. Patrick talks to them by phone and his ex-boyfriend has discovered some ways to read thoughts of Patrick through his iPhone. The secret business advisors are his cerebral creations, which is a clear example of hallucination.
3) During counseling Patrick was found to jump from one topic to another without completing one topic which is the signs of disorganized speech.
4) Upon studying the case, no catatonic behaviors have been found.
5) No negative symptoms have been mentioned in the presented case.
Criteria B: Social or Occupational Dysfunction

Patrick has a disturbance in interpersonal relationship with his colleagues and his ex-boyfriend. As he feels that his colleagues are jealous of him and are talking at his back about him, on the other hand he says that his ex-boyfriend who left him 6 years ago is invading his life by reading his thoughts through his iPhone. This is a clear indication of disturbed interpersonal relationship.
Criteria C: Duration (Less than 6 months)

Patrick believes the existence of some secret agents who contact him by his iPhone and advise him in regard to his work, for which he has become the favorite employee of his manager. This confers his odd believes and unusual perceptual experiences. These all have been started since last “two months”.
