Most of your experience has been in smaller hospitals outside the metropolitan area. The executive management team made it clear to you at interview that they were interested in effecting change throughout the Clinical Services Division, which is why they were seeking an external appointment to the position. In particular, they were seeking an individual who could establish a cohesive and productive working team.
Shortly after commencing in the position it becomes obvious that much of what is achieved is as a result of ‘who you know’. There are many instances when the usual communication channels or procedures are not followed because of ‘system failures’ which have never been brought to anyone’s attention. When running short of stock medications, for example, many nurses contact the nursing unit manager of the medical ward as she has a ‘contact’ in the pharmacy, rather than working through appropriate channels to correct the problem. Patients in the Coronary Care Unit are routinely admitted directly to the unit rather than through the Admissions Office because it is a slow and tedious process.
When you ask why these problems have never been addressed formally, the nursing unit manager of the medical ward tells you “It has always been this way. I am a very experienced health professional – I know the system and how to get around it to get what I need to do the job properly. I see no reason to change things when I think the system is working well.”

In an emergency evacuation of patients, training plays an important role. Here is an incidence of emergency evacuation of Phillips County Regional Medical Center discussed by Vogt and Sorenson (1999):

Shortly after 1:00 p.m. on Thursday, May 8, 1997, clouds of foul-smelling smoke began pouring from an herbicide and pesticide packaging plant in West Helena, Arkansas. An alert was sounded, employees evacuated, and the West Helena fire department was called. As three firefighters prepared to enter the plant, the chemical compounds exploded, collapsing a solid concrete block wall, and killing all three firefighters. As the odorous smoky cloud drifted away from the plant, authorities ordered residents in a 2-mile area downwind of the plant to evacuate and those in the 2- to 3-mile zone to shelter in . . . The findings indicate that 90% of those that were told to evacuate did so but only 27% of those told to shelter-in-place did so, with 68% opting to evacuate instead. The implications of these findings for emergency managers is that people will likely choose to evacuate when both warnings to evacuate and warnings to shelter are issued to residents in close proximity to each other (Abstract section, para. 1).

The evacuation of the medical center was facilitated by efforts taken 6 months earlier to update the hospital’s evacuation plan and reconfirm support agreements for relocation sites and supplies. At that time of updating the plan, the entire staff had also participated in a mock drill. When the safety officer observed the buildings across the highway being evacuated and started questioning officials on the possibility of also being evacuated, evacuation plans familiar to staff were in place and had been practiced. When the Director of Nursing gave the “Code White” alert (the signal that an evacuation to an off-site facility would follow) staff was amply prepared to move patients (p. 11).

Interviews with staff indicated that all patients that could be discharged were sent home with a physician’s nurse. Another four or five patients (the most seriously ill) were transferred to a hospital about a half hour away. The majority of the patients 11 (approximately 17) in the rehabilitation unit were transferred to a vacant wing of the Crestpark Nursing home accompanied by hospital support staff. Only one maternity patient evacuated to the Phillips College—Community College where the hospital staff had evacuated and opened emergency room services. The evacuation of patients began at 1:40 p.m. and all patients (except for two on ventilators and one being stabilized in the emergency room) were out 55 minutes later. The remaining three patients were evacuated 15 minutes later. A variety of vehicles transported patients—vans, school buses, ambulances, private cars, and mortuary services. As a precaution, National Guard personnel provided security at the evacuated hospital (pp. 11–12).

Did the review address a clearly focused issue?

The main objective of the study was to make an assessment of overall efficacy and consistency of therapeutic action and incidence of side effects and to make a review of published trials of coenzyme Q10 for hypertension. Hypertension poses an increase in the burden of disease globally. The article starts by explaining the rationale behind conducting the study which makes it easier for readers to orient themselves (Parahoo 2014). The researchers indicate that hypertension has increased its threat as a global disease with the main management being the use of various medications. They point out that though they have effectiveness in the reduction of blood pressure there are also various undesirable side effects like cardiac or renal dysfunction depression and coughs.

To introduce their study they note the evidence of Coenzyme Q10 being used as therapeutic treatment in lowering blood pressure and improving glycaemic control. A wide range of recent literature is used by the researchers in their literature review. This is supported by previous literature on management of hypertension in clinical trials by use of CoQ10. In this article Rosenfeldt et al. (2007) conducted a qualitative study with a meta-analysis in 12 clinical trials of 362 patients from three randomized control trials eight open label studies and one crossover study. The main intervention plan for all the patients were the use of CoQ10 in the treatment of hypertension and the expected results were that there would be no adverse side effects when using CoQ10. Therefore through this introduction the research can be termed as valid as it considers the use of CoQ10 as an alternative pharmacotherapy in the treatment of hypertension to lower blood pressure.

The authors also looked for the right type of papers which considered all the aspects required in the meta-analysis. In making a review of published trials the researchers began by carefully searching literature for identification of clinical trials of CoQ10 in hypertension therapy. Using an Evidence Based Medicine review they included a variety of databases like American College of Physicians Journal Club Cochrane Database of Systematic Reviews Cochrane Central Register of Controlled Trials Database of Abstracts of Reviews of Effects Embase Medline search (1966-2005) and PubMed (1966-2005). A consultation of the bibliographies of the selected articles was done and the search strategy including significant key words that had a relation to the trial design was utilized (including random allocation double blind placebo controlled and randomized clinical trial) then cross-linked with terms that have a reference to CoQ10 and other terms like blood pressure and hypertension. The studies found address the review question adequately as all the key terms in the question are included in the search and thus in the studies found. Therefore the studies included were relevant to the study objectives.
