Describe a method for providing both the patient and family with education and explain your rationale.Mr. P is a 76-year-old male with cardiomyopathy and congestive heart failure who has been hospitalized frequently to treat CHF symptoms.

Mr. P is a 76-year-old male with cardiomyopathy and congestive heart failure who has been hospitalized frequently to treat CHF symptoms. He has difficulty maintaining diet restrictions and managing his polypharmacy. He has 4+ pitting edema, moist crackles throughout lung fields, and labored breathing. He has no family other than his wife, who verbalizes sadness over his declining health and over her inability to get out of the house. She is overwhelmed with the stack of medical bills, as Mr. P always took care of the financial issues. Mr. P is despondent and asks why God has not taken him.

Question
Considering Mr. P’s condition and circumstance, write an essay of 500-750 words that includes the following:
• Describe your approach to care.
• Recommend a treatment plan.
• Describe a method for providing both the patient and family with education and explain your rationale.
• Provide a teaching plan (avoid using terminology that the patient and family may not understand).

Case Study

You are a registered nurse working at the Early Childhood Clinic with a view to commencing the Child and Family Health Nurse educational pathway. It is your 6th week and you are growing in confidence every day. This morning, the Clinical Nurse Consultant suggests you take charge of the next case and report back to her.

Emma’s Story: A 34-year-old woman Emma presents with her four-year-old daughter Lucy, for examination. Emma is six months pregnant with her third child and has a partner Michael, who is often away on contract work. Emma’s family live interstate and she has been in the area for only 6 weeks. Michael’s retired parents live locally, are supportive and Emma has a good relationship with them. Emma is worried about Lucy because she is a ‘fussy eater’ and would like you to weigh her. She also asks you what immunisations Lucy will need in preparation for starting school.

On examination Lucy’s height is 105cm and her weight is 15.5kgs. A head-to-toe physical assessment finds no abnormalities.

Emma then tells you about her 14-year-old son Josh, who is about to start Year 9. She discloses that she is anxious about him fitting in at the new school. Josh misses his close friends as well as his father Michael, who is currently working away in another state (WA) on contract. Michael isn’t due back for three months but is hoping to eventually find work locally.

Emma looks tired and during your interactions with her she becomes teary and talks about her fears. With the new baby coming, she is worried about how she will cope with Lucy, who can be very energetic, oppositional and hard to control.

Assessment: Discuss the above case study and demonstrate support for healthy families from the perspective of the role of the Registered Nurse.

The discussion MUST be supported with evidence (preferably academic journal articles…referencing page numbers). You MUST include:-

1. Consideration of Lucy and Josh’s physical, cognitive and psychosocial development.
2. A description of the developmental milestones you would expect for a 4-year-old girl and a 14-year-old boy to have achieved.
3. A family assessment including consideration of the factors influencing Emma and her family.
4. Identified potential problems for Emma and her family.
5. Description of the required family-centred nursing care including communication/interviewing strategies that will assist with relationship building with Emma and her family.
6. Meetings and referrals you would arrange with the multidisciplinary team and other appropriate health care professional/agencies.
Instructions:

* Use academic writing that is consistent with essay format, including an introduction and a conclusion.
* Harvard referencing – using in text citing page numbers.
* Preferably Journal references used as evidence and not textbooks.

Paper , Order, or Assignment Requirements

QUESTIONS BY THE PROFESSOR THAT NEED TO BE ANSWERED:

Nurses can contribute to the policies and procedures that are created based on the nursing tasks we do. Being that nurses are at the bedside 24/7/365 days a year, nurses are at the forefront of clinical care.

Nurses see what works, what doesn’t and what can be done better. Using this information, implementing practices, in addition to quality Evidence Based Practice (EBP) research nurses can make a safer environment and level of care for our patients.

Organizations need to allow time for nurses to implement EBP! (Keep in mind that I work as a Correctional Nurse that is employed by the largest Jail System in the United States of America, which is the Los Angeles County Sheriff’s Department Medical Services Bureau.)

Do you feel this time is provided?
Why? How? or Why not?
How does a nurse contribute to nursing research in your current nursing setting (CORRECTIONAL NURSING SETTING)?
How do you utilize research through EBP in this setting (CORRECTIONAL NURSING SETTING)?
What is a problem you see in your current clinical practice that could be addressed by an EBP project (AFRICAN AMERICAN MALE INMATES NOT USING PROTECTIVE SAFE SEX MEASURES WHEN ENGAGING IN SEXUAL ACTIVITIES WITH OTHER MEN WHILE INCARCERATED)?
What are some solutions you would propose for the problem?

How might standardized nursing language (SNL), interprofessional communication, information system support, staff and patient education, organizational culture, and state and national regulations influence technology use? ◦What are the complexities in using technology across a continuum of care?
◦What are the opportunities?

Overview

Write 3–4 pages describing the effective use of patient-care technologies, communication systems, and information systems across the care continuum of a health care system of your choice. Add a one-page executive summary your organization could use to disseminate these ideas.

Because a lack of knowledge when using technology can lead to errors in patient care, effective use of technology in health care is paramount to providing a safe health care delivery environment.

By successfully completing this assessment, you will demonstrate your proficiency in the following course competencies and assessment criteria:

Competency 3: Explain the use of information management tools and technologies to monitor and improve health care delivery and patient outcomes. ◦Describe effective use of technology across the care continuum in a health care setting.
◦Describe how to manage change and technology to improve positive outcomes.

◦Support description of effective patient care with current nursing and informatics theoretical ideas.

Competency 4: Communicate in a manner that is consistent with expectations of a nursing professional. ◦Write coherently to support a central idea in appropriate format with correct grammar, usage, and mechanics.

Questions to Consider

To deepen your understanding, you are encouraged to consider the questions below and discuss them with a fellow learner, a work associate, an interested friend, or a member of the business community.

How might standardized nursing language (SNL), interprofessional communication, information system support, staff and patient education, organizational culture, and state and national regulations influence technology use? ◦What are the complexities in using technology across a continuum of care?
◦What are the opportunities?

◦What are the barriers?

How can we manage change and technology use to develop improved patient outcomes?

How can positive psychology further advance a field of interest (or a career you are exploring, i.e. healthcare), a need in society (such as nourishing our green environment) or a need in the world (such as peacekeeping in the middle east)? A shortcut to this topic is: “Considering positive psychology’s aim to enhance human flourishing worldwide, how can the field make a significant impact on promoting healthy environments and institutions?” We want you to think big and dream, using empirically-based constructs of positive psychology to support your vision. An example: “Individuals, homes, businesses, and communities are working to support environmentally friendly living. Positive psychology can help by…” (And then describe the ways in which PERMA, and/or selfregulation, and/or goal-setting, etc. can support this effort). Please do not focus on achievements that we have already discussed in class (i.e. the work of Dweck in education,).For this paper you must cite at least 2 academic articles from thesyllabus as well as at least one additional source, which could be an additional article, lecture, or video from the syllabus.I posted some of the readings from class on the additional materials. If you don’t find them useful you can look for more readings in the syllabus and ask me to send them to you.Again please cite at least 2 academic article from the course as well as at least one additional source. You may NOT use The Brain That Changes Itself, but it could be used inaddition.

What do you believe is the greatest achievement that the health care industry has made from both the patient and provider perspectives?
MUST be Substantive with All Questions Answered

Healthcare Administration

Objectives for this Assignment:

Tabulate the historical development of healthcare delivery systems in the U.S., and evaluate the forces involved.

The purpose of the Discussion Board is to allow students to learn through sharing ideas and experiences as they relate to course content and the DB question. Because it is not possible to engage in two-way dialogue after a conversation has ended, no posts to the DB will be accepted after the end of each unit.

The U.S. health care system has undergone many changes in its history. Complete the following for this Discussion Board:

What do you believe is the greatest achievement that the health care industry has made from both the patient and provider perspectives? Why?
Do you believe that there is too much or too little redundancy in our health care delivery system? Where does most of the redundancy occur? Where should we have more redundancy (second opinions)?
In your own words, post a substantive response to the Discussion Board question(s) and comment on other postings. Your response should address the DB question(s) and move the conversation forward. You will be graded on the quality of your postings, including mastery of the concept as well as critical thinking. If asked for your opinion, do not simply state that it is a good or bad idea; elaborate on your reasons and argument. Include enough detail to substantiate your thinking as well as your position on the questions or comments.

PLEASE DISCUSS HOW ACCESS TO A VARIETY OF MEDICAL AND PUBLIC HEALTH SERVICES HAS CHANGED….OR NOT CHANGED….INCIDENT TO THE ENACTMENT OF THE ACA. HOW HAS THE LAW ENCOURAGED OR DISCOURAGED CERTAIN BEHAVIORS?

j how access to a variety of medical and public health services has changed….or not changed….incident to the enactment of the ACA. How has the law encouraged or discouraged certain behaviors?
A. Please differentiate between the duty owed to patients presenting to a hospital or other facility by whether their condition is emergency or not and if the provider is pubicly owned or not.
B. Who is ultimately legally accountable for the patient outcomes in a very busy urban Level III Emergency and Trauma Center, such as Shock Trauma in Baltimore?
Part 2
During this period, we consider a number of vital areas, touching as much on personal values as legal issues, indeed reflective of societal religious and cultural norms:
1. Who sets your “moral compass” as a health care professional vs. as a private person? Should these standards be identical? What if they are not?

2. Should the Maryland State Board of Nursing Examiners (Licensing) be implementing the Holy Gospel or the Bible as its ethical and/or moral basis? If not, why?

3. When does the law permit health care providers to ignore the wishes of a patient regarding treatment? If you are ignoring the patient’s wishes (or following them against sound medical judgment), what might you ponder, legally?

4. In reviewing patient privacy expectations vs. public health requirements, how are we balancing competing interests? Consider the stigma presumably associated with certain mental health or infectious diseases and the interests of society to be protected from arguable harm; how would you balance these needs?

Part 3
Please review the 3 items attached and discuss:
1. Can you identify any legal issues arising out of more modern medical approaches, such as telemedicine, electronic health records and data mining for large patient cohorts?
2. Distinguish between civil and criminal liability for the same or similar conduct.
3. Discuss your approach towards questionable activity which may or may not give rise to fraud. What of honest mistakes which are none the less illegal?
4. Define the QUI TAM laws……how can YOU benefit?m
