Staining Helps Identify an Infectious Agent Case Study

A 94-year-old woman went to her local hospital emergency department in mid-November 2001 complaining of a 5-day history of weakness, fever, nonproductive cough, and generalized myalgia (muscle aches). Otherwise, for a person her age she was fairly healthy, although she did suffer from chronic obstructive pulmonary disease, hypertension, and chronic kidney failure.

On physical examination, her heart rate was above normal and she had a fever of 102.3°F

(39.1°C). The rest of her physical examination was normal. Initial laboratory studies of blood cell count, blood chemistries, and chest X ray were also normal except for the chemical urine testing. This finding along with the fever suggested an infection, so the patient was admitted to the hospital. Samples of blood and urine were sent to the microbiology laboratory and set up appropriately.

The next day, microscopic evaluation of the urine culture revealed rod-shaped bacteria that stained red, and the blood culture revealed rods that stained purple. The liquid blood culture was then transferred to appropriate solid media. This finding in the blood was unusual, so a sample culture was sent to the state health department laboratory. Antibiotic therapy was adjusted, yet the patient’s condition deteriorated. Her most serious symptoms localized to her chest, and she was transferred to the intensive care unit. Four days after admission, the health

department announced that the bacteria found in the patient’s blood were Bacillus anthracis.

She was suffering from inhalation anthrax. Further testing showed these bacteria to be of the same strain that had been involved in the recent bioterrorist attack. Despite treatment, the patient died on the fifth day after admission.

What techniques and equipment are used when the bacteria are observed as being purple and red? How are these findings reported?
What are the stages of processing a blood sample Using the 5 I’s of culturing?

Evaluation

Identify explicit and implicit assumptions (values/beliefs) underlying the theory. On what assumptions does the theory build?
Examine if the theory has a description of the four concepts of the nursing metaparadigm. If so, how are they explained in the theory? If the metapardigm is not explained, what elements do you see as relevant to the theory and why?

Compare the main various forms of medical group practice.Physician practices are a key component of the U.S. healthcare system.

According to the text, a significant amount of revenue that the healthcare industry generates can be directly linked to the care that physicians provide for their patients. In addition, physicians in a medical practice add to this revenue stream by admitting patients to a hospital, prescribing prescription medication, ordering home health services and medical equipment, and referring their patients to other healthcare providers for care and treatment. As a future healthcare administrator, it is important for you to demonstrate an understanding of the challenges and opportunities that today’s physicians face in providing quality healthcare services.

Write a four page (4) paper in which you:

Compare the main various forms of medical group practice. Next, select the form that would be most attractive to a newly licensed physician. Justify your selection.
Analyze how the role of the physician in a medical practice has changed in the past twenty (20) years. Assess three (3) specific challenges that today’s physicians face as members of a medical group practice. Support your response.
Suggest three (3) specific competencies that a physician should demonstrate to be successful as a practice manager. Next, determine three (3) hurdles that a physician might face as the leader of a group practice. Provide rationale for your response.
Assess the value of effective Human Resources Management (HRM) in a medical practice. Suggest three (3) functions of HRM necessary to attract and retain the type of employees needed for the practice in order to achieve long-term success. Support your recommendations with specific examples of how each function impacts the overall success of the practice.
Determine three (3) aspects of consumer behavior that the physician’s practice management should consider as part of an effective marketing strategy for medical practices. Provide a rationale for your response.
Use at least three (3) quality ac

Locate and read three scholarly research articles on the role of leadership in managing quality initiatives in healthcare

Option #1: Personal Leadership Style
Utilizing the CSU-Global library, locate and read three scholarly research articles on the role of leadership in managing quality initiatives in healthcare. Select articles that demonstrate three unique styles of leadership (e.g., servant, transformational, and transactional).
Then write an essay that briefly summarizes the three articles and how they relate to healthcare leaders. Describe the leadership theory from one of your selected articles that most closely reflects your current (or future) leadership style and compare/contrast it to another style you have seen used (or read about) by leaders in healthcare settings from these articles. What would you change? What impact does this leadership style have on the healthcare environment? How would you go about changing or developing your leadership style?

Your paper should meet the following requirements:

Be 3-4 pages in length, not including the cover or reference pages.
Be formatted according to the CSU-Global Guide to Writing and APA.
Provide full APA references for articles selected along with associated in-text citations. The CSU-Global Library is a good place to find these references.
Utilize headings to organize the content in your work.

Given the new role of hospitalists in patient care, identify some issues raised by this new brand of physician for the primary physician and patient.
Answer the following questions:

1. The Flexner Report of 1910 is described by the authors as “an accurate and searing description of abuses in the medical schools.” Describe some of the major shortcomings of medical education cited by the Flexner report and the corrective measures that resulted.

2. Academic medical centers were formed by the union of medical schools through affiliations with hospitals that provided clinical experience for medical students. These affiliations were later broadened into academic health centers. Describe the components of the academic health centers and their significance to the evolution of the university-based health professional education complex.

3. The health care delivery system now with ACA mandates, places increased emphasis on maintaining wellness and on promoting disease avoidance through healthy behaviors and lifestyles. What are some challenges this new orientation poses for our existing system of medical education and training?

4. In what ways did physicians’ relationships with hospitals change as a result of health care industry reforms? Discuss the consequences of these changes for physicians’ roles and responsibilities in relationship to hospitals.

5. The dispute over exposing the comparative performance of physicians on a wide spectrum of variables has been resolved in favor of the consumers of health care with requirements such as “Physician Compare.” A number of states have passed legislation that gives the public access to physician information, including disciplinary records, malpractice actions, and whether a physician has lost hospital privileges. Are these fair and balanced bases on which consumers should make decisions about their choice of physicians?

6. Given the new role of hospitalists in patient care, identify some issues raised by this new brand of physician for the primary physician and patient.
