Case Study 1: Statistical Thinking in Health Care Due Week 4 and worth 150 points

Read the following case study.

Ben Davis had just completed an intensive course in Statistical Thinking for Business Improvement, which was offered to all employees of a large health maintenance organization. There was no time to celebrate, however, because he was already under a lot of pressure. Ben works as a pharmacist’s assistant in the HMO’s pharmacy, and his manager, Juan de Pacotilla, was about to be fired. Juan’s dismissal appeared to be imminent due to numerous complaints and even a few lawsuits over inaccurate prescriptions. Juan now was asking Ben for his assistance in trying to resolve the problem, preferably yesterday!

“Ben, I really need your help! If I can’t show some major improvement or at least a solid plan by next month, I’m history.” “I’ll be glad to help, Juan, but what can I do? I’m just a pharmacist’s assistant.” “I don’t care what your job title is; I think you’re just the person who can get this done. I realize I’ve been too far removed from day-to-day operations in the pharmacy, but you work there every day. You’re in a much better position to find out how to fix the problem. Just tell me what to do, and I’ll do it.” “But what about the statistical consultant you hired to analyze the data on inaccurate prescriptions?” “Ben, to be honest, I’m really disappointed with that guy. He has spent two weeks trying to come up with a new modeling approach to predict weekly inaccurate prescriptions. I tried to explain to him that I don’t want to predict the mistakes, I want to eliminate them! I don’t think I got through, however, because he said we need a month of additional data to verify the model, and then he can apply a new method he just read about in a journal to identify ‘change points in the time series,’ whatever that means. But get this, he will only identify the change points and send me a list; he says it’s my job to figure out what they mean and how to respond. I don’t know much about statistics — the only thing I remember from my course in college is that it was the worst course I ever took– but I’m becoming convinced that it actually doesn’t have much to offer in solving real problems. You’ve just gone through this statistical thinking course, though, so maybe you can see something I can’t. To me, statistical thinking sounds like an oxymoron. I realize it’s a long shot, but I was hoping you could use this as the project you need to officially complete the course.”

“I see your point, Juan. I felt the same way, too. This course was interesting, though, because it didn’t focus on crunching numbers. I have some ideas about how we can approach making improvements in prescription accuracy, and I think this would be a great project. We may not be able to solve it ourselves, however. As you know, there is a lot of finger-pointing going on; the pharmacists blame sloppy handwriting and incomplete instructions from doctors for the problem; doctors blame pharmacy assistants like me who actually do most of the computer entry of the prescriptions, claiming that we are incompetent; and the assistants tend to blame the pharmacists for assuming too much about our knowledge of medical terminology, brand names, known drug interactions, and so on.” “It sounds like there’s no hope, Ben!”

Week 10: Ethical Issues in Health Care Delivery Every day nurses make decisions that have critical ethical and legal implications. In addition, nurses must follow doctors’ orders and defer to the wishes of patients and families even when they are not in agreement with those actions. A study conducted by Pavlish et al. (2011) highlights this tension. Researchers surveyed 91 nurses attending a nursing ethics conference and found many of the nurses’ concerns centered on relieving pain in patients while still respecting the wishes of the family (Pavlish et al., 2011). One such dilemma was described as follows: Family of 40-year-old terminally ill patient with cancer refuses all pain medication for fear patient will die.

Nursing and non-nursing theories and models have long been used to drive health promotion and clinical decision making. When advanced practice nurses first begin their

clinical practice, they often select a particular theory or model that most aligns to their personal and professional beliefs—just as you have been asked to do prior

to each Practicum Experience in this program. However, practitioners frequently find that they might be using ideas from multiple theories and models. There is no

right or wrong way, but rather the idea is to select theories or models that can guide your practice and help you influence change within the patient population you

serve. In this Discussion, you select one nursing or non-nursing theory or model and consider how you would use the theory or model to influence change in patient risk

behaviors.

To prepare:
Review this week’s media presentation, as well as the Glanz et al. text in the Learning Resources.

Select a theory/model to use for the health promotion of patients in clinical settings.

The theory/model you select may be a nursing theory/model or a non-nursing theory/model that can be applied to nursing practice.

Reflect on how the theory/model you selected might guide your clinical practice.

Think about how you would use this theory/model to influence changes in a patient’s risk behaviors.
Discuss the theory/model you selected to use for the health promotion of patients in clinical settings.

Then, explain why you selected this theory/model, and discuss how it might guide your clinical practice.

Include a specific example of how you would use this theory/model to influence changes in a patient’s risk behaviors.

Evaluate both theories using the evaluation criteria provided in the Learning Resources. Determine which theory is most appropriate for addressing your clinical practice problem. Summarize why you selected the theory. Using the propositions of that theory, refine your clinical / practice research question.

As addressed this week, middle range theories are frequently used as a framework for exploring nursing practice problems. In addition, theories from other sciences, such as sociology and environmental science, have relevance for nursing practice. For the next few weeks you will explore the use of interdisciplinary theories in nursing.

This Application Assignment, due in Week 10, asks you to evaluate two middle range or interdisciplinary theories and apply those theories to a clinical practice problem. You will also create a hypothesis based upon each theory for an evidence-based practice project to resolve a clinical problem.

NOTE: This Application will serve as your Major Assessment for this course.

To prepare:
• Review strategies for evaluating theory presented by Fawcett and Garity in this week’s Learning Resources.
• Select a clinical practice problem that can be addressed through an evidence-based practice project. NOTE: You may continue to use the same practice problem you have been addressing in earlier Discussions and in Assignment #4.
• Consider the middle range theories presented this week, and determine if one of those theories could provide a framework for exploring your clinical practice problem. If one or two middle range theories seem appropriate, begin evaluating the theory from the context of your practice problem.
• Formulate a preliminary clinical / practice research question that addresses your practice problem. If appropriate, you may use the same research question you formulated for Assignment #4.
You will continue working on this Application in Weeks 8, 9, and 10 using material presented in those weeks to consider interdisciplinary theories that may be appropriate for exploring your practice problem and research question.
To complete:
In a 8- to 10-page paper (including references):
• Briefly describe your selected clinical practice problem.
• Summarize the two selected theories. Both may be middle range theories or interdisciplinary theories, or you may select one from each category.
• Evaluate both theories using the evaluation criteria provided in the Learning Resources.
• Determine which theory is most appropriate for addressing your clinical practice problem. Summarize why you selected the theory. Using the propositions of that theory, refine your clinical / practice research question.
