Case
Study #1:Failure
to adequately assess and monitor the patient post operatively resulting in the
patients death
NOTE: There were multiple co-defendants in this
claim who are discussed in this scenario.
While there may have been errors/negligent acts on the part of other
defendants, the case, comments, and recommendations are limited to the actions
of the defendant; the nurse.
The
decedent/plaintiff was a 67 year old male who underwent a right total knee
replacement. Following the procedure,
the plaintiff was treated in the post-anesthesia care unit where an epidural
catheter was inserted for postoperative pain management.
Following
one episode of hypotension which was treated successfully with ephedrine, the
plaintiff was discharged to an inpatient medical-surgical care nursing unit
with the epidural in place. Although the
defendant nurse customarily worked on the post-acute critical care unit, she
had been re-assigned to the medical-surgical nursing care unit. The defendant nurse stated that she
understood her assignment at the time of the plaintiffs admission to this unit
was to provide oversight of the patient care on the entire floor for that
shift.
The
defendant nurse assessed the plaintiff upon his admission to the unit and found
him to be stable. The defendant nurse
understood that the direct care of the plaintiff was assigned to a c-defendant
licensed practical nurse (LPN).
Approximately three hours after arriving on the unit, the plaintiff was
unable to tolerate ordered respiratory therapy due to nausea and vomited
shortly thereafter. According to the
defendant nurse, approximately ten minutes after the episode of vomiting, the
LPN found the plaintiff cyanotic and unresponsive and immediately called a
code.
The
defendant nurse responded, as did the code team, and the plaintiff was
intubated and transferred to ICU. This
account of events was disputed by the LPN and two other staff on the unit who
understood that the defendant nurse was responsible for the direct care of the
plaintiff.
The
LPN stated that it was the defendant nurse who found the plaintiff to be
unresponsive at some point after the episode of vomiting and called the code
herself. The elapsed time between the
episode of vomiting and the code is also disputed. The eventual diagnosis was anoxic
encephalopathy due to the time that elapsed before CPR was initiated. The prognosis was poor and life support was
withdrawn. The plaintiff breathed
independently and was transferred to hospice care where he subsequently
expired.
Ordered
vital signs and checks of the xyphoid process were not documented. The fact that the plaintiff had experienced
hypotension in the recovery room should have warranted even closer
observation. The episode of nausea and
vomiting should have resulted in additional observation and notice to the
physician.
Resolution
Experts determined that the defendant
nurse had breached the standard of care.

Jessica Williamson is a 28 year old women with a past history of epilepsy. Two weeks ago Jessica was out socially drinking alcohol at a dance club.

Jessica experienced a seizure whilst
on the dance floor. Upon arrival at the local Emergency Department paramedic staff reported that she had experienced a full tonic-clonic seizure that lasted for three
minutes during which she lost consciousness. Jessica was assessed and admitted. During her hospitalization Jessica was
stabilized and discharged from hospital 1 week after the episode. Her discharge medications included an anticonvulsant.
On discharge Jessica was asked her GP for follow-up.
Medical History
Jessica was diagnosed with epilepsy in late adolescence. Initially the seizures were very difficult to control using anticonvulsants but her condition stabilised 5
years ago. Three months ago
she weaned herself off her anticonvulsants as she wanted to fall pregnant. During her hospitalization she was recommenced on her anticonvulsant.
Four years ago Jessica was diagnosed with Type 2 Diabetes Mellitus. Jessica was taught to
control her blood glucose levels using diet and oral hypoglycaemics this is now decreasingly effective. Her most recent observations include:
BP: 150/90
P: 115 regular
Height: 158 cms
Weight: 122 kgs
BGL 12.9
Her current medications are:
Epilim1 gm BD
Metformin 850mg BD
Social History
Jessica lives in a two-storey house with her current partner of six years Martin. Jessicas parents live overseas where her mother has been posted for work. Jessica
is an only child. She works as a
real estate agent selling homes and covers a large geographic area involving long and irregular work hours. This can involve her driving long distances and for long
periods of time. Jessica enjoys her work and the challenges of being independent at work and at home.
Despite being advised by her GP that she must not drive after suffering this recent seizure Jessica continues to do so.
Current Issue
Following the follow-up appointment with the GP the GP was concerned about Jessicas
seemingly poor acceptance of her medical diagnoses and her compliance with the treatment regime for both her diabetes and epilepsy. Jessica indicated she did not need
any more prescriptions for her anticonvulsants at the moment.
Jessica is not particularly careful about monitoring her blood sugar levels. She says she often forgets and tends to check when she feels fuzzy.
As this is your first visit to Jessica in her home justify and discuss the immediate plan of care for her.
Assessment Focus
You are the Registered Nurse from the Community Nursing Service.
You will be undertaking an initial visit to Jessicas home.
Prioritise and provide a rationale for the activities you will undertake as part of this initial visit
and identify two (2) priorities for your follow-up visit.
The purpose of this essay is for students to present a comprehensive discussion linking the health issues identified through a client assessment to the impact they
have on the client
and their level of function in the community setting. It requires the student to prioritise the health issues and to show analysis of the complexity of care provision
demonstrating critical reflection on practice.

For this assignment, using the situation given below, apply the S.W.O.T. Situational Planning Strategy which was described in Chapters 4 and 7 of your course text:

Situation: You are the C.E.O. of Community South Medical Center, a large, urban for-profit healthcare facility. This institution has comprehensive health services including acute care, residential care, independent living, in-home nursing, hospice, neonatal, and pediatric services, advanced cardiac services, a major trauma center, a center of excellence for pulmonary services, and a neurosurgery center.

Community South Medical Center has a positive bottom line and is striving to enhance health services for the population it serves. The patient care mix has shifted in the last three years and its reimbursement source is currently: 25% Medicaid (up 20%); 35% Medicare (down 12 %); 25% employer sponsored health insurance (unchanged), 8% managed care (down 20 percent), 4% private pay (unchanged) and 4% no-pay (up 25%).

The Medical Center is in an older urban area. Businesses with well-paying jobs have gradually been replaced by smaller shops and other small businesses. Overall, the community is trending towards a predominantly elderly population.

The Medical Center has an excellent reputation and has been recognized in the past by receiving a Baldrige Center of Excellence evaluation and a The Joint Commission (T.J.C.) approval for their quality of services; however, recent self-inspections indicated a slight decline in compliance with T.J.C. standards. Patient satisfaction survey results have an overall mean of 95 percent. The medical staff strongly supports new program development and there is an abundant supply of physicians.

Currently, the major issues confronting the Community South Medical Center are: an identified shortage of clinical staff including registered nurses, respiratory therapists, and medical technologists; non-interfacing information technology systems; antiquated facilities and infrastructure; decline in T.J.C. compliance on internal audits; and a shift to its financial mix.

Action: As the C.E.O., it is time to embark on the annual updating of the organization’s strategic plan. The last major revision to the strategic direction was completed three years ago.

In a four- to six-page (excluding title and reference pages) double-spaced paper, consider and respond to the following points:

Select one of the needs of Community South Medical Center and identify at least one strength, one weakness, one opportunity, and one threat (S.W.O.T.) for that need. How do each of these (e.g., the strength, the weakness, the opportunity, and the threat) correlate to the overarching issues confronting Community South Medical Center?
Is it time to revise the strategic mission and plan of the organization from three years ago or continue as originally planned with a yearly assessment? Should the C.E.O. address the issues confronting Community South Medical Center or wait another year? If so, what should the strategic plan be? If not, why?
What leadership positions would be a part of your strategic management team? Why? What would each of their responsibilities entail?
What are some of the market trends you can identify in this area? How might they affect Community South Medical Center? How would you prepare for them?
You must use a minimum of four scholarly sources, excluding the course text, cited according to APA

Identifying approximately two (2) issues requiring specific specialty nursing care for
your patient.

Select two (2) patients/clients with common health problems/diseases or undergoing common
procedures in your specialty(SURGICAL NURSING). Prepare a specialty nursing assessment and care case study for each of
these patients, using the tables provided (appendix C). This is not an essay, you must be succinct when
completing the tables and support the information presented with evidence-based literature.
The focus of this assignment is on the specific assessment and care undertaken by a specialty
nurse in your setting. This will include seemingly basic elements of nursing care, but the purpose of
this assignment is for you to focus on the specific elements pertinent to your specialty. Ask yourself:
what is the focus of my nursing assessment and care as a specialist nurse for patients with these
specific specialty problems/procedures?
You may choose to focus your assessment of the patient at any point of their journey whilst in your care.
Approximately 2500 words (1000 words each case study +
introduction & conclusion) +/-10%

What to include in your assignment:
• Briefly introduce your overall assignment and what you will cover (one paragraph)
• Introduce the first case study patient (and family where appropriate) by stating their name/s
(pseudonym) and health problem/procedure and reason for needing nursing care in your
specialty setting (one paragraph)
• Nursing Assessment: Complete the table (appendix C) regarding specialty nursing assessment
required for this patient in your specialty setting, including:
specialty assessment for the patient in the specialty (select from§ physical,
psychosocial, sociocultural or ethico-legal assessment areas)
normal / expected assessment findings§
reason for any differences in findings for this patient§
nursing strategy required to address this assessment finding§
all citing evidence-based literature§
• Nursing Care: Complete the table (appendix C) regarding specialty nursing care required for this
patient in your specialty setting by:
o Identifying approximately two (2) issues requiring specific specialty nursing care for
your patient.
o Making a nursing diagnosis (or specific statement) about each specific issue requiring
care.
o Describing three (3) key specialty nursing actions to implement, for each of these
issues.
o Evaluating and/or analysing the rationale (supported with evidence-based literature) for
each action.
• Discuss the significance of the specialty nurse role in relation to assessment and care for this
patient in your specialty (one paragraph)
• Evidence-based literature sources must be cited for each element of these tables
• Then repeat the above for an additional patient
• Make an overall conclusion to your assignment and what you have covered (one paragraph).

Explain the 8 steps of hypothesis testing. When performing the 8 steps for hypothesis testing, which method do you prefer; P-Value method or Critical Value method? Why?

This week you will begin working on Phase 3 of your course project. Using the same data set and variables for your selected topic that you used for phases 1 and 2, add the following information to your analysis:

Discuss the process for hypothesis testing.
Explain the 8 steps of hypothesis testing
When performing the 8 steps for hypothesis testing, which method do you prefer; P-Value method or Critical Value method? Why?
Claim: The average age of all patients admitted to the hospital with infectious diseases is less than 65 years of age. Test the claim using α = 0.05 and assume your data is normally distributed and σ is unknown.
Based on your selected topic, perform and document the following steps:

Write the null and alternative hypothesis symbolically and identify which hypothesis is the claim.
Is the test two-tailed, left-tailed, or right-tailed? Explain.
Which test statistic will you use for your hypothesis test; z-test or t-test? Explain.
What is the value of the test-statistic? Show your calculation
What is the P-value? Explain how you determined it
What is the critical value? Explain how you calculated it
What is your decision? Reject the null or do not reject the null? Explain why you made your decision including the results for your p-value and the critical value.
State the final conclusion in non-technical terms

Please show your work for the construction of the test-statistic and explain your process for finding the p-value and critical value. You may use the Equation Editor to format your calculation steps on the Word document or attach an Excel spreadsheet clearly showing each calculation for #B4 and #B5, but your answers still need to appear on the Word document. Also, be sure to number each response in your paper using the same numbers as above
