Quality Improvement in the Health Care Organization Accreditation

The mandate for improving the way in which health care is delivered was stimulated by the public outcry over the estimated 98,000 deaths because of medical errors each year, according to the Institute of Medicine in 1999. Since then, health care organizations have sought means by which the public can be reassured that they were meeting quality and safety standards. Accreditation agencies (e.g., The Joint Commission) and quality awards provide a means for the public to evaluate where the agency is meeting minimum standards.

You have been assigned by your manager to determine which accrediting agencies or quality improvement programs your hospital will utilize in its upcoming revenue cycle. Your hospital is a magnet hospital in a large urban area that provides multilayered services. You have previously used The Joint Commission for your accreditation but feel that you might be better served by using another accrediting body. You have three months in which to gather data and present the information to your manager.

Complete the following for this assignment:

Choose 3 quality improvement or accreditation-related programs to consider in replacing The Joint Commission for your organization, and briefly describe them. Your agency accepts Medicare and Medicaid payments; therefore, you will need to explore, as background, the conditions of participation for Centers for Medicare and Medicaid Services (CMS). This is important information because you will need to compare your list of accrediting agencies and quality improvement programs with the conditions of participation to see if they meet the criteria.
Analyze the costs and benefits of each quality improvement or accreditation-related program by stakeholder group (e.g., patient, provider, and third-party payer).
Rank your quality improvement or accreditation-related program suggestions with rationa

Macronutrient Intake

Write a two to three page essay (double-spaced) assessing your average dietary intake of the macronutrients (carbohydrates, fats, and proteins) based on the computer analysis of your three-day food records. Respond to ALL of the following questions to guide you through your write-up. Refer to “Macronutrient Equations” document to help with your calculations.

2. Your second paragraph should focus on your carbohydrate intake and should answer the following questions: (2 points)
How many grams of carbohydrate did you consume for the three-day average?
How many carbohydrate Calories does this represent (1 gram of CHO contains 4 Calories)

How does this percentage compare to the DRI’s?
If your intake is substantially outside the range recommended, what specific foods could you add or delete from your diet to bring your total into line with the recommendations?
3. Your third paragraph should focus on dietary fiber intake. (2 points)
Why is consumption of adequate amounts of dietary fiber important?
How many grams of dietary fiber did you consume for the three-day average?

Write ten interview questions that will bring out the information you need.
Assignment 1: Interview Preparation and Discussion

Refer to the Course Project Overview in the left navigation area. Per the Course Project, you have selected a mental disorder to work on for your course project. As part of your course project, you will conduct field research on the practice related to your selected disorder. In your field research, you will interview a mental health professional to learn about the practice related to diagnosing and treating the disorder. In this assignment, you will prepare for the interview.

Provide a brief introductory paragraph identifying and discussing the DSM disorder you have selected (furnish appropriate APA citations and References).
Write ten interview questions that will bring out the information you need.
Formulate questions that ensure an appropriate understanding of the disorder. These questions should assist in differentiating the chosen disorder from those found within and outside its parent category.
Tips for approaching your future Interview with a Mental Health Professional

Ideally, you will select a mental health professional in your local area whom you can interview in person. However, if this is not possible, you may choose to select a mental health professional who is located in another city or state. Some students who live in remote areas may choose to contact state or national mental health-related organizations to find potential interviewees. Such organizations include:

The National Alliance on Mental Illness (NAMI): https://www.nami.org/
Mental Health Treatment Facility Locator (SAMHSA-sponsored):http://findtreatment.samhsa.gov/MHTreatmentLocator/faces/addressSearch.jspx
Veterans Administration Hospitals Locator (U.S. Department of Veterans Affairs): http://www.va.gov/directory/guide/home.asp?isflash=1
The Salvation Army’s Harbor Light Centers Locator:http://www.salvationarmyusa.org/usn/harbor-lights
In that case, you would conduct a phone interview.

Ensure that the person you interview has a background in the disorder you are studying.

Case Study

Project description
In a short essay (500-750 words), answer the Question at the end of Case Study 1. Cite references to support positions.

Prepare this assignment according to the APA guidelines found in the APA Style. An abstract is not required.

Case Study 1
Ms. A. is an apparently healthy 26-year-old white woman. Since the beginning of the current golf season, Ms. A has noted

increased shortness of breath and low levels of energy and enthusiasm. These symptoms seem worse during her menses. Today,

while playing in a golf tournament at a high, mountainous course, she became light-headed and was taken by her golfing

partner to the emergency clinic. The attending physician’s notes indicated a temperature of 98 degrees F, an elevated

heart rate and respiratory rate, and low blood pressure. Ms. A states, “Menorrhagia and dysmenorrheal have been a problem

for 10-12 years, and I take 1,000 mg of aspirin every 3 to 4 hours for 6 days during menstruation.” During the summer

months, while playing golf, she also takes aspirin to avoid “stiffness in my joints.”

Laboratory values are as follows:
Hemoglobin = 8 g/dl
Hematocrit = 32%
Erythrocyte count = 3.1 x 10/mm
RBC smear showed microcytic and hypochromic cells
Reticulocyte count = 1.5%
Other laboratory values were within normal limits.

Question
Considering the circumstances and the preliminary workup, what type of anemia does Ms. A most likely have In an essay of

500-750 words, explain your answer and include rationale.
Please Include:
Accurate identification of anemia classification and rationale. :- Identification of anemia classification and rationale

is offered in detail, while demonstrating evidence of deeper insight and/or reflection.
Explanation of patient diagnosis with rationale from case findings. Outside sources and/or medical and nursing references

used to support conclusions: Explanation of patient diagnosis uses outside sources and/or medical and nursing references

to support conclusions and is offered in detail, while demonstrating evidence of deeper insight and/or reflection.

Why can’t Joseph’s brain simply replace those neurons that were damaged and destroyed by the stroke?Joseph is a 59-year-old construction worker who recently suffered a stroke.

Read the following case study:
Joseph is a 59-year-old construction worker who recently suffered a stroke. A small blood clot became lodged in one of the vessels serving the right side of his brain and restricted blood flow to a portion of his right precentral gyrus. Many neurons in that area were damaged or destroyed and, as a result, Joseph is partially paralyzed on the left side of his body. He is able to move his left leg and walk, and can also move his left arm, but his left hand and the left side of his face are paralyzed.
Answer the following questions about the case study:
Why were Joseph’s left face and hand paralyzed, but not the rest of his arm or his leg?
What is preventing Joseph from moving his left hand? In your answer, include the following:
First, describe normal function of a motor neuron that forms a synapse with a muscle cell. How is the signal that initiates movement transmitted from the neuron to the muscle? Describe this process in detail.
Next, keeping in mind that the motor neuron itself is not damaged, explain how damage or destruction of a neuron that communicates with the motor neuron prevents Joseph from moving his hand.
With time, and perhaps some physical therapy, Joseph may recover some of the movement in his left hand and face. What factors might contribute to this recovery? In your answer, include the following:
Why can’t Joseph’s brain simply replace those neurons that were damaged and destroyed by the stroke?
How might compensation allow Joseph to regain some of his lost movement?
Describe at least one experimental treatment that might improve movement in Joseph’s case, and explain how this treatment would impact Joseph’s nervous system.
