nursing reflection
Order Description
submit three reflective pieces written in the style of blogs. The blogs are to reflect on your professional growth during clinical placements over the course of your degree and also focus on your soon to be transition from student nurse to professional nurse in practice. As this is a reflective piece it is to be written in the first person. Gibbs Reflective Cycle will be provided as a template to write the reflective piece. You may use any other reflective model of your choosing. Academic writing standards apply; correct spelling and grammar are essential however references are not. Expected word count approximately 500 words per blog.
The blogs will be saved as one document and then uploaded to Mahara. In Mahara you will create a folder and submit the folder using a secret URL.
For ease of marking please do not use blogging websites for this assessment.
You may wish to post your blogs to the designated discussion forum when it opens in Week 7 to promote peer feedback review and collegial discussion around key concepts.
The blogs are to be addressed in this order
Blog 1
Write a reflective piece on potential challenges experienced during your past clinical placements that may impact on this final placement. OR if you are on placement write a reflective piece on challenges you are currently experiencing.
Blog 2
Use the experiences identified in Blog 1 and write a reflective piece identifying these and any other potential challenges that may arise during your transition period when you are working as a Registered Nurse.
Blog 3
Identify and reflect on your strengths and use these to provide strategies to address challenges you identified in Blog 2.

Prepare a 2-3 page report describing, comparing and/or contrasting the specific standards that apply to the authentication, dating and timing of physician orders, and make a recommendation as to what the policy for your hospital should be.

Your hospital is reviewing its medical staff rules and regulations. As the Director of the HIM department, you have been asked to work with the physician Chairperson of the Health Information Committee to make a recommendation regarding a policy on authentication, dating and timing of physician orders, including verbal, telephone, electronic, and handwritten orders. The policy you develop must allow the hospital to be in compliance with the documentation requirements of the Joint Commission, CMS Conditions of Participation and the State Licensing regulations.

Requirements:

Research and examine the documentation requirements in each of the following standards related to physicians’ orders for an acute care inpatient facility, including authentication, dating, and timing of orders:
Illinois State licensure rules (www.ilga.gov)

Click on Subpart L Records and Reports
Click on and Review Section 250.1510 Medical Records
CMS Conditions of Participation

Review the CMS Conditions of Participation provided by your instructor.

The JCAHO standards

Review the Joint Commission Standards provided by your instructor.

RC.01.01.01
RC.00.02.01
RC.01.03.01
Prepare a 2-3 page report describing, comparing and/or contrasting the specific standards that apply to the authentication, dating and timing of physician orders, and make a recommendation as to what the policy for your hospital should be.

In order to be in compliance with more than one set of standards the hospital must comply with the strictest standard of the three.

Example of One Way to Approach the Assignment Regarding Verbal Orders

List each standard that applies and then select the strictest to include in your policy.

Joint Commission Standard RC.02.03.07: Qualified staff receive and record verbal orders.

Elements of Performance for RC.02.03.07 3. Documentation of verbal orders includes the date and the names of individuals, who gave, received, recorded, and implemented the orders

State Licensure Rules (IL) Section 250.1510 Medical Records b)Organization 5c)

Authentication of Medical Record Entries 1) All entries into the medial record shall be authenticated by the individual who made or authorized the entry.

CMS Conditions of Participation A-0454: All orders, including verbal orders, must be dated, timed and authenticated promptly by the ordering practitioner …

Analysis of the Standards: CMS has the strictest policy of the three as it requires not only dating and signing but timing as well.

Recommended Policy: All verbal orders must include the date and time of the verbal order and the name of the individual physician who gave the verbal order. All orders must be authenticated by the ordering physician within 24 hours.

We have the capacity, through our dedicated team of writers, to complete an order similar to this. In addition, our customer support team is always on standby, which ensures we are in touch with you before, during and after the completion of the paper. Go ahead, place your order now, and experience our exquisite service.

Discuss what types of medical approaches (ECT, prescription medications, psychosurgery, or current medical devises) you would recommend for the patient by referring to studies showing its effectiveness in treating the disorder.

To begin your Assignment, do a search of the Kaplan Library and the internet. You are also to use course material, but your project should include at least four additional references from the library and/or the internet (your final Assignment must include one library resource). The internet sources you use for the Assignment should be credible. You should not use internet sites such as Wikipedia where anyone can post information or About.com. Your information should be from acceptable psychological or medical societies, such as the National Institute of Health, Web MD, etc.

You will address the following areas that are outlined in the patient portfolio document. Part 2 through Part 4 of the patient portfolio should be a minimum of 675 words. All work should be in the student’s own words with quotes used very sparingly. No more than 10% of the work should be direct quotations. Be sure to address each of the issues with the use of several sources in the form of in-text citations to support your answers, and use proper APA format, including Times New Roman 12-point font. For help with APA writing style, please refer to the APA Quick Reference on Course Home and the Kaplan Writing Center (accessible from your student home page).

Using the provided template, as-is, develop the patient’s portfolio by addressing the following:

Part1: Background information about the patient

Provide background information that includes (a) the patient’s disorder, and (b) a list of the diagnostic criteria that the patient meets listed in the DSM-5 under the diagnostic criteria for the disorder.

Part 2:Psychological Approach

Select a psychological theory from the course textbook (chose either a cognitive or behavioral theory). Refer directly to the textbook or an academic source to explain the theory. After explaining the theory, apply it by discussing how it may explain the development of the case study’s mental illness. (Approximately 225 words)

Part 3: Biological Approach

Select a theory from the biological approach from the course textbook (choose either the genetic or neurological influence). Refer directly to the textbook or an academic source to explain the theory. After explaining the theory, apply it by discussing how it may explain the development of the case study’s mental illness. (Approximately 225 words)

Part 4:Sociocultural Approach

Select a theory from the social and cultural or interpersonal relationship approach from the course textbook. Refer to the textbook or an academic source to explain the theory. After explaining the theory, apply it by discussing how it may explain the development of the case study’s mental illness. (Approximately 225 words)

Part 5: Treatment

Discuss what types of medical approaches (ECT, prescription medications, psychosurgery, or current medical devises) you would recommend for the patient by referring to studies showing its effectiveness in treating the disorder. (Approximately 75 words)
Compare and contrast the side effects the patient may experience from the selected type of medical approach and support what benefit the treatment has on the brain chemistry or neurotransmitter activity. (Approximately 75 words)
Discuss psychotherapy options (e.g., cognitive behavioral therapy, group therapy, exposure therapy). Explain the chosen therapy and specifically how the patient would benefit from it. (Approximately 100 words)
Provide both short and long-term goals for the patient’s treatment plan. Include accomplishments or behavioral changes you want to see in the patient. (Approximately 75 words)
Part 6: Conclusion

Defend which of the approaches can best explain the development (or cause) of the case study’s mental disorder and why your chosen treatment plan would be the most beneficial plan for the patient (biological approach, psychological approach, or sociocultural approach). (Approximately 150 words)

For this assessment you will design and develop a project prospectus in which you propose an evidenced-based practice change quality improvement or innovation that demonstrates the synthesis of WGU graduate competencies. This project is not primary research but should focus on scholarly evidence in support of practice improvements innovation or general practice change. It will not require an institutional review board (IRB) approval. You will be required to conduct a comprehensive literature review that defines the evidence in support of your project. The literature review represents a collection of primary research studies that have already been conducted and that provide guidance for best practices. Once your prospectus is complete you will create a professional presentation that outlines your project prospectus and then present it to your stakeholder group and preceptor. Upon successful completion of this assessment you will progress to Task 2 of the Field Experience.
Before you start your proposal you will first need to secure organizational approval for your capstone project and obtain a letter of agreement from your preceptor. In Task 1 you will be required to develop a capstone proposal that describes your problem and proposed solution. You will not be permitted to begin the implementation of your project until the project has been approved by WGU.
Requirements
A. Submit a signed approval letter from the organization where your project will be conducted using the attached Organizational Approval Letter Template.
B. Submit a signed copy of the attached Preceptor Agreement.
C. Summarize your proposal by doing the following:
1. Identify the problem.
2. Describe why the problem you have identified requires a practice change quality improvement or innovation.
3. Explain the causes of the problem.
4. Identify the stakeholders.
a. Discuss each stakeholders interest power and influence in relation to the project.
D. Explain the purpose of your project.
E. Explain your proposed solution for your project.
F. Provide an evidence summary to support your proposed change.
1. Provide a reference list for your evidence summary in APA format that includes a minimum of five scholarly peer-reviewed sources that were published within the last five years.
G. Explain your proposed implementation plan which includes the following:
1. Plan of action
2. Timeline
3. Needed resources and personnel
4. Proposed change theory
5. Barriers to implementation

What is physician-assisted suicide, and how does it differ from active and passive euthanasia?
[a] On January 1, 2014, Brittany Maynard was diagnosed with brain cancer. In April, she was given just six months to live.

Brittany wanted to have the option of ending her own life with the help of her doctor before the cancer became unbearable. So, she moved to Oregon to take advantage of the state’s ‘Death with Dignity’ law. On November 1, 2014, surrounded by family members, Brittany ended her own life by ingesting drugs prescribed by her doctor.

Oregon is currently one of five states in which physician-assisted suicide is legal. In June of this year, physician-assisted suicide became legal in California (the fifth state).

Do you morally approve or disapprove of California’s decision to legalize physician-assisted suicide?

Your paper should carefully address the following questions:

1. What is physician-assisted suicide, and how does it differ from active and passive euthanasia?
2. Would J. Gay-Williams approve of physician-assisted suicide? (Why/why not?)
3. Would James Rachels approve of physician-assisted suicide? (Why/why not?)
4. Do you approve of California’s decision to legalize physician-assisted suicide? (Why/why not?)

—————————————————————————————————————————-

[b] Brittany Maynard suffered immensely as a result of her brain cancer. And (presumably) she believed that death would bring an end to her suffering.

Do you believe that Brittany’s death (or being dead) is a bad thing for her?

Your paper should carefully address the following questions:

1. Would Stephen Rosenbaum & Epicurus believe that Brittany’s death was bad? (Why/why not?)
2. Would Lucretius believe that Brittany’s death was bad? (Why/why not?)
3. Would Tom Nagel believe that Brittany’s death was bad? (Why/why not?)
4. Do you believe that Brittany’s death was bad? (Why/why not?)

**Your paper should be 4 typed, double-spaced pages and is due on the day of the Final Exam (Wednesday, September 7th). Please email your paper to me at bjohns@csusb.edu. Late papers will be penalized two letter grades (e.g., A → C) for each day beyond the due date.

HCP reviews the nursing hx in the EHR and pt first assessment while he is placing order in chart, he is given an electronic alert by clinical information system to remind him that he may want to order a MRI of the brain to R/o CVA. HCP cancels alert because he believes that observation of apand re-eval her in a.m. Will suffice. As he walks on floor the next morning, nurse signals him frantically to come and see Mrs. jeng. It appears that she has a major CVA. HCP knows that this may have happened with or without Mrs. Jeng having an MRI the day before, but he’s concerned about the ramifications of the cancelling the alert day before. a.) Second part of the essay has to deal with ”cultural baggage”. Five patterns that each society has in common are Free Education for all, Free health care for all, Race does not matter, Welfare is only for people that truly need it, and religion of your choice.
