Ben Davis had just completed an intensive course in Statistical Thinking for Business Improvement, which was offered to all employees of a large health maintenance organization. There was no time to celebrate, however, because he was already under a lot of pressure. Ben works as a pharmacist’s assistant in the HMO’s pharmacy, and his manager, Juan de Pacotilla, was about to be fired. Juan’s dismissal appeared to be imminent due to numerous complaints and even a few lawsuits over inaccurate prescriptions. Juan now was asking Ben for his assistance in trying to resolve the problem, preferably yesterday!

“Ben, I really need your help! If I can’t show some major improvement or at least a solid plan by next month, I’m history.”
“I’ll be glad to help, Juan, but what can I do? I’m just a pharmacist’s assistant.”
“I don’t care what your job title is; I think you’re just the person who can get this done. I realize I’ve been too far removed from day-to-day operations in the pharmacy, but you work there every day. You’re in a much better position to find out how to fix the problem. Just tell me what to do, and I’ll do it.”
“But what about the statistical consultant you hired to analyze the data on inaccurate prescriptions?”
“Ben, to be honest, I’m really disappointed with that guy. He has spent two weeks trying to come up with a new modeling approach to predict weekly inaccurate prescriptions. I tried to explain to him that I don’t want to predict the mistakes, I want to eliminate them! I don’t think I got through, however, because he said we need a month of additional data to verify the model, and then he can apply a new method he just read about in a journal to identify ‘change points in the time series,’ whatever that means. But get this, he will only identify the change points and send me a list; he says it’s my job to figure out what they mean and how to respond. I don’t know much about statistics — the only thing I remember from my course in college is that it was the worst course I ever took– but I’m becoming convinced that it actually doesn’t have much to offer in solving real problems. You’ve just gone through this statistical thinking course, though, so maybe you can see something I can’t. To me, statistical thinking sounds like an oxymoron. I realize it’s a long shot, but I was hoping you could use this as the project you need to officially complete the course.”

“I see your point, Juan. I felt the same way, too. This course was interesting, though, because it didn’t focus on crunching numbers. I have some ideas about how we can approach making improvements in prescription accuracy, and I think this would be a great project. We may not be able to solve it ourselves, however. As you know, there is a lot of finger-pointing going on; the pharmacists blame sloppy handwriting and incomplete instructions from doctors for the problem; doctors blame pharmacy assistants like me who actually do most of the computer entry of the prescriptions, claiming that we are incompetent; and the assistants tend to blame the pharmacists for assuming too much about our knowledge of medical terminology, brand names, known drug interactions, and so on.”

In what way does the public health department work with the Centers for Disease Control and Prevention (CDC) in providing information on local outbreaks and receiving information from the CDC on possible concerns for their geographic regions?

Answer: The CDC works with partners at the local, state and national level to monitor and prevent disease outbreaks (including bioterrorism), implement disease prevention strategies, and maintain national health statistics. The agency also leads public health efforts to prevent and control infectious and chronic diseases, injuries, workplace hazards, disabilities and environmental health threats (Lurie, Wesserman, Stoto & Myers, 2004). A good example would be the whopping cough and measles outbreak in WA a couple of years ago. The CDC assisted with disease surveillance, which helped create a preparedness and containment strategy for healthcare organizations around the state.

Answer: A registry is a list of patients and their relevant clinical data that can be sorted by a condition or set of conditions in order to improve and monitor the care of the population. Healthcare organizations that have a well-organized disease registry are more prepared when it comes to controlling future outbreaks since they have data they can look at and create strategies from.

Four years ago she undertook genetic testing after presenting to her primary care physician with complaints of a breast lump. She was found to have a BRCA-1 gene mutation, which makes her high risk of developing breast cancer and a relative risk for developing ovarian carcinoma. At only 33 years of age, my vibrant aunt who works as a community social worker has had a generally unremarkable health history. She has had countable hospital visits in her adult age during most of which she was treated as an outpatient for minor ailments. Family history is significant of our grandmother who died from cardiorespiratory arrest secondary to pulmonary thromboembolism. She was on treatment for stage-4 breast cancer that was diagnosed at age 45.Having a BRCA-1 gene mutation carries up to 85% risk of developing carcinoma of the breast and other cancers as well including ovarian carcinoma (Rebbeck, 2015). The disease is majorly diagnosed in women though carcinoma of the male breast has also been described with prevalence not well elucidated. The disease often presents as a breast lump that may have been picked during a routine breast exam or incidentally. The lump is often painless and danger signs include rapid growth, irregular borders with a suggestive history such as that of breast cancer diagnosed at an early age in a first-degree relative (Broek, 2015). Other known risk factors include nulliparity, early menarche and delayed menopause, estrogen treatment, smoking among others.As alluded to earlier, having a BRCA-1 gene mutation also predisposes her to ovarian carcinoma. This condition can be easily averted if my aunt decides to have a bilateral oophorectomy (Broek, 2015). This is an example of a modifiable risk factor that exists for her. More studies are being done to understand breast cancer and particularly the link of genetics to its development. However, as it stands, the risk is well defined for BRCA-1 AND BRCA-2 mutations. (Rebbeck, 2015).

In regards to President Obama’s Patient Protection and Affordable Care Act (Public Law 111-148, 2010) or ACA, the goal is to offer all Americans affordable health care, health insurance reform, improve patient resources and reduce hospital and operation cost.

The goal of this project is to ensure that Gotham Hospital continues to operate efficiently while meeting President Obama’s new ACA criteria and continue to provide quality care to its patient community. The team plans to offer several recommendations that will ensure Gotham is in compliance with ACA/ACO, as well as compare and analyze the effects of Pre and Post ACA hospital operations and elaborate on the hospital’s concern about how it can reduce the cost of chronic diseases particularly diabetes using its disease management program.

The project outcome is simple; to increase enrollment/utilization of the diabetes disease management and prevention at Gotham Hospital taking into account its diverse uninsured/underinsured population. The team will assess the current diabetes program at the hospital, offer recommendations and design a proactive program that will help increase utilization of diabetes care services at the Gotham Hospital.

Progress Healthcare, a local hospital in South Carolina, recently installed a nurse tracking system. This Real Time Locating System (RTLS) from Stanley Healthcare is an expansion of the commonly known Aeroscout, which is used for asset tracking. The goal is to understand workflow streams and utilize the data to streamline the workflows through a LEAN event. Data is captured from the employee badges and sent wirelessly to a local server and then sent to an online database for business intelligence analysis and reporting. Progress Healthcare also sends patient monitoring information to the floor monitoring server and then to the electronic medical record (EMR). The monitoring data is also stripped of personal health information (PHI) and sent to the same Internet server to correlate tracking information with each room. Understanding staff movement is seen by leadership and management as efficient. It is also a way to justify chargers and to validate the level of care a patient receives. Many of the staff nurses and assistants see it as a distraction and as another time consuming measure that diminishes patient care.

Henrietta Lacks was an African American woman whose cancer cells were the source of the HeLa cell line, one of the most important cell lines in medical research. HeLa cells are “immortalized cells.” Immortalized cells lines are important because they will reproduce indefinitely under specific conditions. The HeLa immortal cell line was vital for creating the polio vaccine, cloning (i.e., Dolly the sheep), gene mapping (i.e., the Human Genome Project) and more.
Mrs. Lacks was the unwitting source of these cells when her tumor was biopsied in 1951 during treatment for cervical cancer at Johns Hopkins Hospital in Maryland. Her cells were then cultured by George Otto Gey, who created the cell line known as “HeLa” (i.e., Henrietta Lacks). Consent was NOT obtained to culture her cells, nor was she (or her family) ever compensated for the use of the cells despite the fact that the HeLa cell line revolutionized modern medicine!
