Identification, discussion, and documentation from the literature of your perspective on the basic four metaparadigms/concepts of patient, nurse, health, and environment.
Two Practice-Specific Concepts: Identification, discussion, and documentation from the literature of your perspective on at least two other concepts specific to your own practice.

Model answer

Basic Concepts in Nursing
In Orem’s theory, four nursing metaparadigm concepts are described; person, environment, health, and nursing. After a keen analysis of the theory, the perspective of the 4 concepts is bound to change. The image of the ‘person’ is confined to one who receives nursing care after self-care administration fails. The ‘environemnt’ view includes the developmental environment and ‘health’ encompasses health maintenance and health promotion. In addition, ‘nursing’ encompasses a comprehension of the significance of the nursing theory as well as its application on the practice settings and nursing profession’s development (Wold, 2012).
Person
According to Orem, the person is the patient; an individual that functions symbolically, socially, and biologically. The person has a capacity to develop and learn, for self-knowledge, and making deliberate actions so as to satisfy the self-care requirements for maintaining health. If a person is not in a position to care for himself, a family member or care provider can do so. On the same note, individuals cannot escape the natural forces (Wold, 2012). People benefit from nursing since they experience health-related limitations which inhibit continuous self-care. When an individual is sick, the nurse should be committed towards doing all she can to treat the patient even if in collaboration with other professionals.
Patients should be treated as intelligent persons who have an intense capacity of learning regarding how to cope with their condition. They should also be treated as so since in many healthcare setups, many professionals ignore this. There is a great need for patient education and communication in relation to self-care administration and provision of healthcare should only be secondary. In the past, patients were treated as the passive recipients but Orem’s theory promotes a shift to self-empowerment and the ‘person’ to enhance self-care (Kelly, 2012).
Nursing
People usually seek nursing services because of diseases that interfere with the ability of continuous self-care. Caring for oneself is not inherent but learned through integrity and development. The nurse has a critical role of assisting individuals to meet self-care and therapeutic needs. Orem’s theory can be applied in rehabilitation centers that evaluate patient outcomes and discharge plans (Wold, 2012). The theory can also be applied in developing and guiding practice, and both quantitative and qualitative methodologies in nursing research.
Health
Healthcare is presently a great concern regarding the increasing costs. This calls for alternative care methods. There is a need to ignore the traditional approach where the patient was passive and engage a more proactive recipient who is ware and in a capacity to make decisions that impact his health positively. Self-responsibility is a key aspect of health promotion. To Orem, this is not a novel concept since she states that people can develop their psychomotor and cognitive skills, in addition to the motivation related to actions needed to maintain their health. If patients are keen on self-care, then illnesses can be prevented (Ellis & Hartley, 2012). There is a trend of increased disease prevention and health promotion as opposed to caring for patients predominantly. Nurses are increasingly engaged in approaches that can help people return to well-being and health.
While I had not incorporated the concepts of health maintenance and promotion previously, Orem emphasizes that self-care activities should never be taken for granted. Her health description is very applicable to nursing paradigm in relation to health maintenance. Health status is not a stagnant but an active growth process. There is an emphasis on prevention that promotes self-care (Ellis & Hartley, 2012). During the aging and reproduction processes, there is need for more self-care requirements. Promoting health should be a vital component of every person’s life. This should include eating healthy, being physically active, and a supportive network.

Analyze types of drugs prescribed to treat endocrine and musculoskeletal disorders. The endocrine and musculoskeletal systems play important roles in the regulation and coordination of activities throughout the body
Endocrine and Musculoskeletal System

The endocrine and musculoskeletal systems play important roles in the regulation and coordination of activities throughout the body. When alterations of these systems occur, many patients require long-term drug therapies. In addition to drugs prescribed by their health care providers, patients with these disorders also often take herbal and dietary supplements. According to the American Diabetes Association, 22 percent of diabetic patients use herbal therapy and 31 percent use dietary supplements (American Diabetes Association, 2009). This may impact the effects of prescribed drugs, as well as pose a concern of adverse drug reactions in patients. When treating patients with endocrine and musculoskeletal disorders, it is important to educate patients on associated risks. As an advanced practice nurse prescribing drugs, you must carefully consider all drug, herbal, and dietary therapies prior to finalizing treatment plans.

This week you examine types of drugs prescribed to patients with endocrine and musculoskeletal disorders, as well as the impact of patient factors on the effects of drugs. You also explore ways to improve patient treatment plans including suggested drug therapies. Finally, you examine types of diabetes and the impact of diabetes drugs on patients.

250-WORD DISCUSSION POST DUE TOMORROW

Let’s see who can follow directions:

Work through the simulation titled Resource Allocation from the end of Chapter 8 of your course text. Review the various options in the simulation, then select “Your Own Option” to type out your own solution to the scenario. You will need to copy and paste your response from “Your Own Option” into the discussion board forum as it will not be transferred otherwise. Here is a brief synopsis of the simulation regarding the hospital’s budget and dilemma:

Hospital costs in millions for one year:

One 35-year-old cancer patient who needs significant time with the doctor, medical supplies, tests, and around the clock care. Cost: 100
Emergency Room operations for daily care and treatment of about 100 people (~365,000/year). Cost: 100
2 Senior Patients who need hip replacement surgery. Cost: 50

Who are the federal, state, and local policymakers involved in your chosen policy issue? The student will reflect on the experience, develop plans for follow-up with the policymaker as needed, and consider ways in which advocacy efforts can continue.

Students are to select a policy issue (Healthy Nutrition Class Policy), research the issue including policy analysis, plan the policy, and plan a call or meeting with a policymaker for the purpose of presenting the issue, proposing recommendations and asking questions about how change can occur. The student will reflect on the experience, develop plans for follow-up with the policymaker as needed, and consider ways in which advocacy efforts can continue.

Over the past several decades, the prevalence of overweight and obesity in children has increased considerably. Obesity is a solemn disease. It can be caused by genes, medical problems or bad lifestyle eating habit. As a result, there are some lifestyle measures to preventing these kids from starting on the wrong track. I would like to communicate with my local mayor or local officials and encourage them to initiate healthy nutrition classes from grade 3 to grade 12 to all school children in my local area yearly. I believe the earlier we can start introducing these kids to healthy diet and exercise, the more likely these kids will stay fit and decrease their chances of Diabetes, HTN and high cholesterol later in their lives. I will use incrementalism model that will allow me to use step by step to enhance these kids understanding of the importance of good nutritional habit (Mason and Leavitt). As the children grow with each grade level, the nutrient class gets more challenging and start to incorporate the importance of regular exercising.

Please Use level 2 heading for Introduction, Key Points, Empirical Evidence and Specific Examples. Please use references articles no more than 5 years old with DOI. Please CINAHL or EBSCO database if there is no DOI with article (see below for instruction on guidelines and references).

1) Introduction

Introduction clearly introduces your policy-priority issue (Healthy Nutrition Class Policy), current status, and overview of plan for legislative visit. Concluding statements summarizing content have no inaccuracy.

2) Key Points

Providing a plan/message: Identification of all key strategies of your policy-priority issue plan, ask, and recommendations are clearly analyzed and fully articulated.

1. Review what has been done in the past by others regarding your chosen policy issue(state of Florida). What was the result of their actions related to this policy issue? Why is this issue important to nursing?

2. Who are the federal, state, and local policymakers involved in your chosen policy issue? (Mayor Wayne Messam for the city of Miramar, Florida) How can you contact your policymaker? Do you have phone numbers, addresses, immediate staff contacts, and more? Be sure you single out a policymaker whom you know is interested in your issue.

3. What will be the plan for your legislative visit? When, where, and how?

4. What is the message you want to give to your selected policymaker/legislator? Can you present a compelling ‘story’? Can you convey your passion and experience with the policy issue? Can you present basic research data in an easy-to-understand and interesting way? What is your ‘Ask’? What are you recommending? Please review the examples of a policy brief in your text or on the American Nurses Association website. What are your expectations of the policymaker and for your visit in general?

3) Empirical Evidence

Recommendation: Empirical evidence supporting your policy- priority issue is clearly critiqued in a comprehensive and accurate manner. What specific supportive evidence do you have for this issue, including evidence and references? You will need to include this in your message/ask/recommendation(s).

1. How do you plan to convey your message? What considerations must you have in place with respect to time constraints, availability of policymaker, and contingency plans?

4) Specific Examples

DIRECTIONS

Use the following Case Scenario, Subjective Data, and Objective Data to answer the Critical Thinking Questions.
Case Scenario
Mrs. J. is a 63-year-old woman who has a history of hypertension, chronic heart failure, and sleep apnea. She has been smoking two packs of cigarettes a day for 40 years and has refused to quit. Three days ago, she had an onset of flu with fever, pharyngitis, and malaise. She has not taken her antihypertensive medications or her medications to control her heart failure for 4 days. Today, she has been admitted to the hospital ICU with acute decompensated heart failure.
Subjective Data
Is very anxious and asks whether she is going to die.
Denies pain but says she feels like she cannot get enough air.
Says her heart feels like it is “running away.”
Reports that she is so exhausted she cannot eat or drink by herself.
Objective Data
Height 175 cm; Weight 95.5 kg
Vital signs: T 37.6 C, HR 118 and irregular, RR 34, BP 90/58
Cardiovascular: Distant S1, S2, S3 present; PMI at sixth ICS and faint; all peripheral pulses are 1+; bilateral jugular vein distention; initial cardiac monitoring indicates a ventricular rate of 132 and atrial fibrillation
Respiratory: Pulmonary crackles; decreased breath sounds right lower lobe; coughing frothy blood-tinged sputum; SpO2 82%
Gastrointestinal: BS present: hepatomegaly 4 cm below costal margin
Critical Thinking Questions
What nursing interventions are appropriate for Mrs. J. at the time of her admission? Drug therapy is started for Mrs. J. to control her symptoms. What is the rationale for the administration of each of the following medications?
IV furosemide (Lasix)
Enalapril (Vasotec)
Metoprolol (Lopressor)
IV morphine sulphate (Morphine)
Describe four cardiovascular conditions that may lead to heart failure and what can be done in the form of medical/nursing interventions to prevent the development of heart failure in each condition.
Taking into consideration the fact that most mature adults take at least six prescription medications, discuss four nursing interventions that can help prevent problems caused by multiple drug interactions in older patients. Provide rationale for each of the interventions you recommend.
