What’s the difference: LTC vs. Acute

ABC Medical Center is building a long term care (LTC) facility (nursing home) that will be connected to the hospital. You have been invited to a planning meeting with the HIM Director who must plan for the patient record system in the new facility. The HIM Director has asked you to prepare a PowerPoint presentation on the differences between an acute care setting and a long term care setting. What are some of the first things you could do to be sure you understand how medical records might be managed differently in this type of facility? What are some of the obvious differences between a hospital and a long term care facility that will likely affect health information at this facility?
Your presentation should be a minimum of 8 slides (including a Title slide and References slide) and address the following:
Overview of an acute care facility versus a LTC facility.
Examples of services that are specific to a LTC facility.
Differences in chart documentation.
Differences in health care payers.
Overview of how medical records might be managed differently in LTC versus an acute care facility.
Include APA formatted citations for your references.

What is the difference between model, theory, framework, and philosophy? How are they related? (Discriminate between the concepts.)

What is the difference between model, theory, framework, and philosophy? How are they related? (Discriminate between the concepts.)

R/ A framework is a “broad overview, outline, or skeleton of interlinked items which supports a particular approach to a specific objective, and serves as a guide that can be modified as required by adding or deleting items (Business Dictionary, 2017). This is the most basic version of the concept or idea. A framework can lead to a model, which leads to a theory. The best way to explain the differences and similarities between a model and a theory is Maslow’s Hierarchy of Needs Theory. A model is a “graphical, mathematical (symbolic), physical, or verbal representation or simplified version of a concept, phenomenon, relationship, structure, system, or an aspect of the real world” (Business Dictionary, 2017). In Maslow’s Theory, the model is the colorful pyramid that we all remember seeing in our first text book in nursing school; it is a simple, and easy to remember, version of Maslow’s theory. However, the theory is the full explanation and clarification of those simplified ideas on the pyramid. In Maslow’s full theory, each level of the pyramid is detailed and explained to further the learner’s understanding of his ideas. Therefore framework, model, and theory have a relationship to each other.

Philosophies and theories are similar, in that they are both guides; and they are different in that they guide differently. A philosophy is “careful thought about the fundamental nature of the world, the grounds for human knowledge, and the evaluation of human conduct (The Philosophy Pages)” (Mastin, 2008). The best example to explain these differences is nursing theory and nursing philosophy. Nursing theory is based upon assumptions regarding nursing care and practice, that are then researched, and then put into practice for a large group, such as a hospital system or nursing university. Whereas, nursing philosophy guides the personal practice of each individual nurse separately.

Assess the health needs of individuals, families, aggregates, and communities using demographic and epidemiological data to identify population health risks.

Caring for Populations Milestone Community Windshield Survey Guidelines
Updated 4/27/2017

Purpose

The purpose of this assignment is to complete a direct observational assessment of a community in your area. This windshield survey will be the initial step in uncovering a community health problem in your area and identifying community dynamics. This problem will be further assessed in the following two assignments.

Course Outcomes

This assignment enables the student to meet the following course outcome.

CO2: Assess the health needs of individuals, families, aggregates, and communities using demographic and epidemiological data to identify population health risks. (PO 4)

Due Date

Submit your completed windshield survey form by 11:59 p.m. MT Sunday of Week 2.

Points: 150 points

Directions

Download the Community Windshield Survey form (Links to an external site.)Links to an external site.. Type your name and answers directly onto this Word document. Your form does not need to follow APA formatting; however, you are expected to use a professional writing style with complete sentences, accurate grammar, and correct spelling. If references are used, they must be cited in the text and the complete reference should be included in the reference box in APA format. References are optional.

Save the form by clicking Save as and adding your last name to the file name; for example, NR443 Windshield Survey Form_Smith.
Submit the completed form by Sunday, 11:59 p.m. MT at the end of Week 2.
Length: The completed form should be three to four pages in length (not including any optional references).
Guidelines

Below are the requirements needed for successful completion of the Windshield Survey Form.

Introduction to the Community: Identify the community you will be using for this assignment with the city and state and provide a brief, one paragraph description of the community. Your community should be the area where you live or the area surrounding your work setting. The community must include a residential area and be broad enough to answer the survey questions. You do not need to include demographic data.
Windshield Survey: Assess your community by doing a windshield survey. Information about the components of a windshield survey is located in your textbook, Nies & McEwen (2015) on page 98 (Box 6-2). Drive through the area and report your observations by describing each of the following six areas using a majority of the questions from each category of Box 6-2 as your guide (one to two in depth paragraphs for each category). Be sure to include what you observed related to each of these categories, and also include any significant items that are missing in your community because this may be equally important in identifying a community health problem.
Community vitality
Indicators of social and economic conditions
Health resources
Environmental conditions related to health
Social functioning
Attitude toward healthcare
Note: It is helpful to conduct this assessment at least two different times: during the day or evening, on a weekday and/or on the weekend. If possible, plan on asking someone to drive during your survey so that you can take notes.

Vulnerable population: Identify an aggregate or vulnerable population in your community based on your observations. What did you observe about this population? See Units five (5) and six (6) in Nies and McEwen (2015) for examples of vulnerable populations and aggregates.
Conclusion: Provide a summary of your findings. Describe several potential community health problems for the vulnerable population that you identified.
References: The purpose of this assignment is to document your observations of your community. Outside sources are not required. However, if sources are used, these sources must be cited within the form (Author, year) and listed in the reference section in APA format (see APA resources for help with formatting).
Best Practices in Completing the Form

The following are best practices in preparing this project.

Make sure all elements of the form are included.
Review directions and grading rubric below thoroughly.
Cite any outside references as above (these are optional).
Use a professional writing style with complete sentences.
Proofread prior to final submission.
Check for spelling and grammar errors prior to final submission.
Abide by the Chamberlain academic integrity policy.

Reflect on an issue or problem that you have noticed in your nursing practice. Consider the significance of this issue or problem.

Course Project: Introduction to Course Project
Evidence-based practice involves a great deal more than simply reading nursing periodicals on a regular basis. Nurses can take a more proactive approach to evidence-based practice by identifying authentic problems and concerns, and then using that to guide their inquiries into current research. In this way, nurses can connect the results of relevant research studies to their nursing practice.
For the Course Project, you identify and apply relevant research to a specific nursing topic or problem. You begin by formulating an answerable question that is relevant to nursing and evidence-based practice. In later weeks of this course, you continue the Course Project by conducting a literature review and then determining how the evidence from the literature can be applied to nursing practice.
Before you begin, review the Course Project Overview document located in this week’s Learning Resources.
Note: This Course Project will serve as the Portfolio Assignment for the course. In addition to submitting portions of this Project in Weeks 2 and 5, you will turn in all three deliverables in Week 10.
Course Project: Part 1—Identifying a Researchable Problem
One of the most challenging aspects of EBP is to actually identify the answerable question.
—Karen Sue Davies
Formulating a question that targets the goal of your research is a challenging but essential task. The question plays a crucial role in all other aspects of the research, including the determination of the research design and theoretical perspective to be applied, which data will be collected, and which tools will be used for analysis. It is therefore essential to take the time to ensure that the research question addresses what you actually want to study. Doing so will increase your likelihood of obtaining meaningful results.
In this first component of the Course Project, you formulate questions to address a particular nursing issue or problem. You use the PICOT model—patient/population, intervention/issue, comparison, and outcome—outlined in the Learning Resources to design your questions.
To prepare:
• Review the article, “Formulating the Evidence Based Practice Question: A Review of the Frameworks,” found in the Learning Resources for this week. Focus on the PICOT model for guiding the development of research questions.
• Review the section beginning on page 71 of the course text, titled, “Developing and Refining Research Problems” in the course text, which focuses on analyzing the feasibility of a research problem.
• Reflect on an issue or problem that you have noticed in your nursing practice. Consider the significance of this issue or problem.
• Generate at least five questions that relate to the issue which you have identified. Use the criteria in your course text to select one question that would be most appropriate in terms of significance, feasibility, and interest. Be prepared to explain your rationale.
• Formulate a preliminary PICO question—one that is answerable—based on your analysis. What are the PICO variables (patient/population, intervention/issue, comparison, and outcome) for this question?
Note: Not all of these variables may be appropriate to every question. Be sure to analyze which are and are not relevant to your specific question.
• Using the PICOT variables that you determined for your question, develop a list of at least 10 keywords that could be used when conducting a literature search to investigate current research pertaining to the question.
To complete:
Write a 3- to 4-page paper that includes the following:
• A summary of your area of interest, an identification of the problem that you have selected, and an explanation of the significance of this problem for nursing practice
• The 5 questions you have generated and a description of how you analyzed them for feasibility
• Your preliminary PICOT question and a description of each PICOT variable relevant to your question
• At least 10 possible keywords that could be used when conducting a literature search for your PICOT question and a rationale for your selections

Please respond to the following questions in your discussion post:

After listening to the podcast, what do you think about the importance of carefully planned research in medicine/health (or psychology?)? What evidence do you have in mind (in other words give an example of something that caught your attention or sounded problematic but could be addressed by an improvement in research methods)?
What are the consequences of poorly executed research when it comes to health or medicine? Do you worry as a patient/consumer?
What is one thing/fact that surprised or maybe even shocked you from this podcast?

The editor asks you to write an 8-10 page feature story about the steps you should take when fraud and abuse cases are reported to a facility. This is very timely as you recently received a call on your hotline regarding a potential fraud and abuse issue. The caller indicated that Dr. Greedy was billing for services that had not been provided. You are in danger of losing reimbursement for Medicare and Medicaid programs if this behavior is not stopped. Your feature should address the ethical and moral components that healthcare providers and healthcare facilities face with fraud and abuse issues. Your research should include the following aspects:How to conduct an investigation. It should include the following elements:Reviewing the initial complaint: What are the items you should look for in a compliant to determine validity?
Notifying the appropriate upper management of the complaint unless they are implicated in the complaint: What are the steps to take to determine who is involved?
Obtaining additional information as necessary and developing a plan for the investigation: What other items are important to the investigation?
Conducting interviews with staff, residents and/or management: Delineate the types of questions to ask in the interview.
Determining if the allegations are substantiated or unsubstantiated: Identify criteria to determine if substantiated or unsubstantiated. How to develop a correction action plan. The plan may suggest:A recommendation for a subsequent audit or follow-up to the complaint and determination of when this is necessary.
A recommendation to refund any overpayments to federal government, insurance company or individual payer and when that may be the best course of action.By Monday, August 17, 2015, deliver your assignment to the M5: Assignment 1 Dropbox.Use the following file naming convention: LastnameFirstInitial_M5_A1.doc.
