Critiquing the validity and robustness of research featured in journal articles provides a critical foundation for engaging in evidence-based practice. In Weeks 5 and 6, you explored quantitative research designs. In Week 7, you will examine qualitative and mixed methods research designs. For this Assignment, you critique a quantitative and either a qualitative or a mixed methods research study and compare the types of information obtained in each. To prepare: 1. Select a health topic of interest to you that is relevant to your current area of practice. The topic may be your Course Portfolio Project or a different topic of your choice. 2. Using the Walden Library, locate two articles in scholarly journals that deal with your portfolio topic: 1) Select one article that utilizes a quantitative research design and 2) select a second article that utilizes either a qualitative OR a mixed methods design. These need to be single studies not systematic or integrative reviews (including meta-analysis and metasynthesis). You may use research articles from your reference list. If you cannot find these two types of research on your portfolio topic, you may choose another topic. 3. When assessing patients for these conditions, it is important to keep in mind that while some female patients might present with typical signs and symptoms, others might present with atypical signs and symptoms that are unique to women. For this Discussion, consider signs and symptoms presented by the women in the following case studies and develop differential diagnoses:

A 48-year-old overweight African American female is in the clinic for a wellness visit. A routine fasting lipid panel returned with the following results:

You are seeing a 63-year-old African American female for a two-week history of intermittent chest pain. The pain varies in intensity and resolves with rest. She does not believe it has increased over time. She is a nonsmoker with a history of hypertension treated with Lisinopril 10 mg once daily. She had an exercise stress test one year ago that was within normal limits. Her physical exam findings are as follows: HR–90, BP–150/92, R–22, O2Sat 98% RA; lungs: clear to auscultation bilaterally; cardiovascular: apical pulse of 90 RRR, no rubs, murmurs, or gallops. Chest wall mildly tender to palpation that reproduces her complaint of pain. Extremities include no clubbing, cyanosis, or edema. The remainder of the exam is within normal limits.

Case Study 3

A 32-year-old Asian American female is in the clinic for a history of recurrent headaches for the past year, occurring monthly, lasting up to 12–18 hours. The headaches are sometimes associated with photophobia, nausea, and vomiting. She takes either acetaminophen or ibuprofen for relief that is not always successful. She uses Ortho Tricyclin for birth control. Her physical exam is within normal limits.

To prepare:

Review Chapter 8 of the Tharpe et al. text and the McSweeney et al. article in this week’s Learning Resources.

Review and select one of the three provided case studies. Analyze the patient information.

Consider a differential diagnosis for the patient in the case study you selected. Think about the most likely diagnosis for the patient.

Reflect on the appropriate clinical guidelines. Think about a treatment and management plan for the patient. Be sure to consider appropriate dosages for any recommended pharmacologic and/or nonpharmacologic treatments.

Consider strategies for educating patients on the treatment and management of the disorder you identified as your primary diagnosis.

By Day 3

Post an explanation of the differential diagnosis for the patient in the case study you selected. Explain which is the most likely diagnosis for the patient and why. Then, based on the appropriate clinical guidelines, explain a treatment and management plan for the patient, including proper dosages for any recommended treatments. Finally, explain strategies for educating patients on the disorder.

He was admitted to the ward later the same day and was noted to have a fever of 39.6, diffuse abdominal pain and abdominal distension.
His blood tests taken on admission also reveal an elevated WBC. Physical Examination: T 39.6, P 110, RR 24, BP 95/65, oxygen saturation 99% in room air. Heart rate is regular and no murmurs are heard. He is lying in bed with his legs drawn up and is very anxious when you try to palpate his abdomen. His pain score using a numerical scale is 7/10.

This assessment task prepares you to access a patient in your care and identify those at risk. Select an ‘at risk’ patient for analysis and write a brief summary of the patient’s relevant health problems and any management provided.
Critically analyse one health problem using evidence based findings relating this to the ‘at risk’ status of the patient. Consider any system failures that may have contributed to the presenting problem and the relationship to communication pathways. The cases you read and discussed in MyLO are examples of an “at risk” patient. Your discussion should include the complexities of health care decision making and its relationship to inter-professional communication pathways.
You must clearly demonstrate the related pathophysiology and management that contributed to the patient’s condition. Laboratory findings should be included if relevant to the case. Expand and justify management by identifying and discussing key nursing and medical interventions, including clinical assessment, physiological monitoring, supportive therapies and evaluation of the care provided. Justify and critique the relevant nursing interventions by drawing on recent, evidence-based literature and propose recommendations for future practice if indicated. These external stressors have the potential to affect the person physically, psychologically, socially, culturally and spiritually in a way which may ultimately compromise healing and recovery. It is imperative that while caring for people who are critically ill, that nurses consider the impact of the environment on the person’s illness, healing, and recovery. An awareness of how the environment affects a person who is critically ill, allows nurses to initiate and incorporate nursing care to support the person through their illness by addressing environmental stressors while undergoing treatment in the critical care setting.
