Case Study:

Ann is a seventy-seven-year-old grandmother. She has one daughter and three grandchildren. Two years ago, she was diagnosed with moderately advanced Alzheimer’s disease, which causes her to have periods of confusion, frustration, anger, and obsessive thinking. When speaking, she is uncertain and her speech patterns are choppy. Before her mental deterioration, Ann was a woman of love, intelligence, and patience. Realizing they were getting older, she and her husband, Frank, discussed their wishes should anything happen to them. She told Frank that if there was no chance of survival, she would not want to be hooked to a breathing machine. They never got around to filling any papers.

Frank is a sweet and sensitive man. His wife’s state frightens him. Ann’s love for him has been the focus of his life for sixty years. His urgent desire for the best care for Ann shows his devotion and love. Their daughter, Sarah, is a businesswoman. She is a hard worker and a good mother. She is forty-five, successful, and intelligent. Although she loves her parents dearly, she lives ten hours away. She regrets not seeing much of them, especially recently. Frank feels his role is to take care of Ann. He has spent the past year with her, watching after her, cooking for her, cleaning the home, and witnessing her deterioration. Finally, Ann is unable to walk alone safely and he finds he must have her admitted to a long-term care facility. He calls Sarah to come and help with the arrangements. After having Ann admitted to a local nursing home, both Frank and Sarah remember the pleading look in Ann’s eyes as they walk away.

After three weeks in the nursing home, Ann starts to cough and run a fever. She is seen by doctors and diagnosed as having pneumonia. She is transferred to a local hospital, where she is given intravenous antibiotics. Although the progress of her pneumonia is halted by the antibiotics, she stops talking and refuses to eat. The physician calls Frank to insert a feeding tube. Frank calls Sarah to ask what to do. They wonder, Is a feeding tube equivalent to a breathing machine? Would it be possible to allow Ann to lie there and die of starvation? Is that murder? What would she want? What is the right thing to do? (Burkhardt & Nathaniel, 2008).

Describe the mental state of the sniper since the three shootings have occurred in which he killed the offenders.

The counselor for a major metropolitan police force. Her job is to interview persons who are referred by superiors, who voluntarily come to her for counseling, or who, because of a specific incident, are mandated by departmental regulations to seek your counsel. On the last four callouts over the past 6 weeks, three of them have resulted in the offender being shot to death by your units sniper. The counselor is scheduled to speak with him this week about his feelings concerning the shootings. The counselor received a call from his wife the night before last. It seems that the sniper has taken to sleeping in the living room and not in his bedroom. His wife has been unable to convince him to sleep in bed with her for the past month. He claims that he has headaches and goes to sleep on the couch. After the last mission in which he had to shoot a suspect, the team went to a local bar that they patronize. The sniper reportedly got drunk and, while hugging another member of the team, he started crying about the three men that he had to shoot over the past six weeks. The next day, he acted like the crying incident did not happen. He either forgot it in the alcohol haze, or pretended not to remember it. In a callout last night, the sniper and his partner took up a position at the rear of a house where home invaders were barricaded. At one point, an offender exited the back door with a gun in his hand. The sniper stood up in the open and walked right up to the offender, who was dazed at the snipers behavior. The sniper struck the weapon from the offenders hand with the butt of his rifle, and then knocked him to the ground, where he placed him in handcuffs. Although it initially seemed to be a brave act, it violated all protocol to break cover, and it risked the sniper, his partner, and the offenders life. You have a decision to make at this point. In a 35 page paper, respond to the following: Describe the mental state of the sniper since the three shootings have occurred in which he killed the offenders. Articulate two of the possible mental disorders that the sniper has that are interfering with his work. Explain whether, if the sniper would get the treatment needed, he might be able to return to his duties as a sniper, or possibly back to regular uniformed duties. Relate how treatment of the sniper would benefit him as an individual in terms of his interpersonal relationships and his own safety. It is critical that, when you make a statement of fact in your presentation, you cite the reference that you used to obtain the information from. Do this in the text of the paper, and then include the reference in your References page. As always, your paper will be submitted in the current edition of APA format. No abstract is required because this is a short position paper, but a title page, reference page, and appropriate running header with page numbers are necessary.

Choosing a community partner for community outreach project

you will need to explain why your list of assets are well suited to your community nursing plan?
1. Your community nursing plan is to reduce morbidity and mortality related to cardiovascular disease by expanding a smoking cessation program.
2. Your assets are the local hospital, local schools and churches, and, and the American Heart Association. How can these partners help your cause? ie. provide space, provide flyers, provide nicotine patches, etc. use peer reviewed nursing journals to support your plan and your use of resources. These journals cannot be older than 2012! no references from 2011, or older are acceptable.
3. state from which partner you will receive what asset. ie., space from schools/churchesl, flyers from American Heart Association, perhaps nicotine patches from hospital or other institutions.
4. Write a script to pitch to potential partners supporting your cause. Use scientific journals to strengthen your position. This script will be the bulk of the paper. You will use the topics in items 1 – 3 to construct your script. Script must present problem “cardiovascular disease” goal – to impact heart disease by reducing the high rate of smoking in the community. Plan – Teaching in schools and churches, to prevent new smokers. Distributing nicotine patches to current smokers and literature for smoking cessation.
The introduction to this paper is a brief description of the community Mott-Haven, Bronx New York, where there is a high level of heart disease and cigarette smoking. This introduction should be brief.
