Concept boxes must be linked using lines and linking words/phrases to demonstrate logical relationships between concepts.

Concept Map (500 words)
• The concept map must be computer generated.
• When researching the topic, use the pathophysiology template that is on the unit vUWS site to assist you in organising your information. The template will also assist in identifying key concepts and their relationships.
• Concept boxes should have titles with relevant content summarised within the box.
• The summarised content may be in point format and size 10 font (Arial or Times New Roman) may be used on the concept map.
• Concept boxes must be linked using lines and linking words/phrases to demonstrate logical relationships between concepts.
• For conciseness, numbers may be used to replace traditional APA referencing within concept boxes. If you use numbers, you must include a reference list in numerical order, corresponding to your references immediately after the concept map.
• Critical thinking and creativity are essential components of the concept map. The map design needs to be visually stimulating.

Guided Questions (1000 words)
Case study
Mrs Brown, a 78 year-old female, was admitted to the Emergency Department at 6am after waking up with severe breathlessness. Her family informed you that the patient has a history of heart failure that had been diagnosed two years ago. On admission to the Emergency Department the clinical manifestations were:
• Severe dyspnoea
• Respiratory rate of 24 breaths/minute
• SpO2 85%, on room air
• BP 170/95mmHg
• Pulse rate of 120 beats/minute • Auscultation of lungs identifies bilateral basal crackles
Mrs Brown was connected to an ECG monitor that showed atrial fibrillation. A diagnosis of acute exacerbation of chronic left-sided heart failure was made.
Question 1. (500 words)
Explain the pathogenesis causing the clinical manifestations with which Mrs Brown presented.

Question 2. (200 words)
Discuss two high priority nursing strategies to manage Mrs Brown and provide evidence-based rationales for these strategies.

Question 3. (300 words)
Two of the drugs that were given to Mrs Brown were IV furosemide and sublingual glyceryl trinitrate.
a. Discuss the mechanism of action of these two drugs, and relate to the underlying pathogenesis of an acute exacerbation of chronic left-sided heart failure. And,
b. Describe the nursing implications (monitoring for and responding to adverse effects, and evaluating therapeutic effect) when administering these two drugs to a patient with an acute exacerbation of chronic left-sided heart failure.

Post a description of the pathophysiology of cardiovascular disorders, including how the factor you selected might impact the pathophysiology.
Cardiovascular Disorders

Veins and arteries are vital elements of the cardiovascular system. They carry the blood supply through the body and are essential for proper function. Sometimes veins and arteries malfunction, resulting in cardiovascular disorders. Malfunctions of arteries and veins are similar to malfunctions of a water hose. Consider the structure and function of a hose. A tap releases water, which then travels through the hose and comes out the other end. If the hose has been dormant for several months, dirt and rusty particles might build up inside, resulting in a restricted flow of water. Similarly, buildup of plaque inside the coronary arteries restricts blood flow and leads to disorders such as coronary heart disease. This disease is one of the most common cardiovascular disorders, and according to the National Heart, Lung and Blood Institute (2011), is the leading cause of death for men and women in the United States. In this Discussion, you examine the pathophysiology of cardiovascular disorders such as coronary heart disease.

To prepare:

Review this week’s media presentation on alterations of cardiovascular functions, as well as Chapter 23 in the Huether and McCance text. Identify the pathophysiology of cardiovascular disorders.

Select one patient factor: genetics, gender, ethnicity, age, or behavior. Consider how the factor you selected might impact the pathophysiology of cardiovascular disorders.

Select one of the following alterations of cardiovascular disorders: peripheral arterial disease, myocardial infarction, coronary artery disease, congestive heart failure, or dysrhythmia. Think about how hypertension or dyslipidemia can lead to the alteration you selected.

By Day 3

Scenario B – Tracey is a 35 year old woman who presents for assessment to the Emergency Mental Health Community Team. She is six months pregnant with her third child. Her other children, to her ex-partner, are aged three and five. Tracey is in a heterosexual relationship with her partner of one year, Peter. Peter moved in to live with Tracey and her children eight months ago. The children’s father is distressed about Peter moving in to live with his children. There is verbal hostility between Tracey and her ex- partner each time he visits to collect the children for a weekend visit. Her expartner frequently states he is making arrangements for court to have full time care of the children. Tracey and her current partner, Peter, are in $35,000 uncontrolled debt after a recent new joint business venture failed. A finance company calls weekly requesting payment. Tracey and her partner stopped making the mortgage repayments three months ago, with the belief that dealing with the uncontrolled debt would be a better option. Her parents are supportive but live in Queensland. Tracey is able to have face to face contact with them once a year. She does chat with her parents over telephone every week if possible. Tracey states she is worried about how she will cope with her two children and new baby. She reports low mood, weekly panic attacks and fleeting thoughts of suicide. During your meeting with Tracey, you note she is fidgety and restless, breathing rapidly, shaking slightly, sweating and tearful. She is not wishing to actively engage with the mental health services, for fear it will impact on her having care of her children, stating she will visit her General Practitioner (GP).

1) Investigate and outline the prevalence/incidence of anxiety (depending on the scenario you have chosen) in Australia – Your answer needs to cover: gender, age groups, specific risk groups, hospitalisations. 225 words

? Please refer to ABS Australia 2007. ( Only Australia statistics)

2) Using current literature discuss TWO (2) factors that may have contributed to the development of the client’s presentation and mental health concerns. 225 words

? Contributed factors: Child custody issues, financial stress, Verbal hostility with ex husband
3) Using current literature discuss TWO (2) ethico-legal issues related to your scenario.225 words
The Mental Health Act (2007) is the legal construct used predominately for clinical care in the mental health services. Please ensure if you are using the Mental Health Act, you refer to the latest NSW version. However you may also wish to consider legal frameworks around access to health records, storage of health records or privacy. Professional nursing standards or ethics literature may also help guide your thinking.

For example you may wish to consider:

• the professional boundaries around confidentiality
• the use of the mental health act for when safety concerns are noted
• a nurse’s role in assessment and engagement with a client knowing that they may have to activate resources to possibly schedule a person and detain them ( ie remove their freedom) due to concerns about their safety
• You may also consider a person’s right to engage with mental health services when there are actual risks or potential risks to their safety or the safety of others
• The ethics of a nurse taking no action if risks to self are identified or potential risks are noted
• a person’s autonomy and right to choose
• a nurse’s discussion with other health professionals ie. GP
• a nurse’s involvement with family members when risks are noted versus a person’s right to choose no family involvement
• a nurse’s professional and ethical role in child protection matters for risks or potential risks
• non-maleficence; a principle to avoid harm. However assessment and interventions may cause distress to the person and their family
4) Identify TWO (2) nursing/midwifery concerns / problems with evidence from your chosen scenario- Your risk identification should be focused on the next 1-5 days of nursing/midwifery care for your client.100-150 words(USE TABLE)
? Symptoms of anxiety
? Inability to cope
? Fleeting thoughts to suicide
? Reducing engagement in services
? For each nursing / midwifery concern / problem you have identified in Question 4, outline and describe TWO (2) evidence based nursing / midwifery interventions (ie; what you would actually do as a nurse / midwife to support the person and how you would do it). Your interventions should be focused on nursing / midwifery care for your client over the next 1-5 days. They must be interventions which you would actually undertake directly with your client within your role as a nurse. You are also required to provide a clear rationale for each nursing intervention (ie. Why have you chosen the particular nursing interventions? How will the interventions support the person or contribute positively to their current presentation or concern?) Each intervention and rationale must be supported by current literature. 225 words (USE TABLE)

Paper details:
submit an essay covering ALL 4 topics mentioned above. I will go into more detail below about each topic. Here is a link to the application requirements if you would like to get a better idea of the hospital and so forth. http://www.cookchildrens.org/SpecialtyServices/Nursing/nurseresidency/critical-care/Pages/default.aspx Here are some ideas related to each topic that I ask you include (in a more formal manner of course though please). 1. I am very interested in the pediatric field of nursing and love the idea of getting to rotate through different areas throughout the residency. Cooks Childrens is a prestigious hospital with lots of acknowledgement and recognition. 2. My goals and expectations are to be confident and comfortable in my nursing skills with the pediatric population. I expect to learn a tremendous amount of information and skills throughout the residency and hope to continue my passion working with children after completion. 3. The two values I ask you to focus on are caring and integrity. Nursing is so much more than getting by doing the bare minimum. I want to focus on going above and beyond for my patients and being an extreme patient advocate. Sometimes its the simplest things that have the greatest impact on a patient. For example just taking the time to sit and talk to a patient that may be lonely or just needs someone to talk to. Nursing is so much more than passing meds and doing a quick assessment it involves emotional psychiatric and physical implications that are specific to each and every individual patient. 4. I have always had a passion for children and would be so blessed and excited to be able to experience such a wonderful program. (something along those lines for this topic please) You can google Cook Childrens Ft. Worth Texas to get a better idea of the hospital and its values and all that! If you have any questions at all please contact me. Please include these details in a more formal manner.

what are the demographics of the people who live there? What types of health services are available?

Write a Final Paper focusing on diversity in healthcare organizations and Homeless people to include military veterans. The focus of your paper must be a subculture of people that exists within your local community. The subculture you chose should represent a minority group. Remember, the term “minority” in this context refers to more than race. For example, depending on the community, a minority subculture could be the poor, a religious group, or other group of people who share a common bond. You will complete this assignment by responding to the following: • Describe the community you have chosen to write about. What are the demographics of the people who live there? What types of health services are available? If you live in a very small community, expand the boundaries of the community to include the hospitals, healthcare providers, etc. used by the people in this area. • Describe the subculture you have identified in this community. Include the values, beliefs, practices, and other aspects of culture that influence the health and health practices of the group you have selected. Identify any specific health problems or health issues experienced by the group. Your description should be based on the literature and may include your own experiences. Are there any health service programs available in the community designed to meet the needs of this population? • If you were to create an “ideal system” that would exemplify culturally competent healthcare for this subculture in your community, what would it be like? Your response should address the community at large and the entire healthcare continuum as it relates to your subculture. For example, you should include ambulatory and preventative services such as doctor’s offices, acute care services such as hospitals, and long term care services such as nursing homes. Write an 8–10 page paper (using APA format) with references.
