Discussion: Models of Interdisciplinary Geriatric Care Teams
With the growing population of frail elders, there is an increase of geriatric patients requiring ongoing care for multiple medical conditions. This creates the need for interdisciplinary geriatric care teams. Often, the dynamics and culture of these teams differ across various sites of care, such as assisted living, home care, hospitals, long-term care, and rehabilitation facilities. As an advanced practice nurse, it is important to understand your role in the care team as well as your potential impact on patient care. In this Discussion, you explore models of interdisciplinary geriatric care teams for different sites of care and the varying roles of the advanced practice nurse.
Consider the following three case studies:
Case Study 1
Mrs. Martinez is an 83-year-old Mexican American widow who lives in her own home and is cared for by her adult daughter. Mrs. Martinez owns the home, and her daughter lives with her and provides the care. Her daughter brought her mother to the clinic today to ask to speak to the social worker. She requests that her mother be placed in a nursing home. The daughter states that her mother has nothing to do during the day. The television is on The Weather Channel most of the day because Mrs. Martinez has limited English capability and is unable to read closed-captioning. Mrs. Martinez also has two sons who do not live in the local area, but they do call regularly and check in with their mother and sister. The two sons are opposed to moving their mother to a nursing home because they had promised her that they would never put her away.
Case Study 2
Mr. Williams, a 79-year-old African American widower, resides in a foster care home. He has lived there for 4 years since his wife died. He is a former minister. His medical history includes long-term diabetes, high blood pressure, and benign prostatic hypertrophy. The home care provider has requested a home visit to evaluate Mr. Williamss ability to remain in the home. The provider states that because Mr. Williamss vision is seriously compromised (he is nearly blind), and because he has been unable to get to the toilet as quickly as necessary (he is very unsteady on his feet), his care is becoming burdensome. According to the home care provider, for safety reasons, Mr. Williams may not fit the criteria for remaining in the foster care home.
Case Study 3
Mrs. Randall is a 77-year-old female who resides in a long-term care facility. She has a history of frequent falls and is severely cognitively impaired. The nursing staff at the long-term care facility called the nurse practitioner at the medical home office to report the recent development of productive cough and high fever. There have been cases of flu in the facility; however, Mrs. Randall has had a flu shot. The nurse practitioner in the office requests a chest x-ray in the long-term care facility. The nurse on duty in the facility states that there is no portable chest x-ray equipment available. She further requests that Mrs. Randall be transferred to the emergency room of the local hospital. Mrs. Randalls daughter has durable power of attorney for health care decisions for her mother. The long-term care facility has notified the daughter of the change in her mothers condition. The daughter says whatever the nursing home wants is fine with her.
To prepare:
Review this weeks media presentation, as well as the American Geriatrics Society and Arbaje et al. articles in the Learning Resources.
Research models of interdisciplinary geriatric care teams that are used at various sites, such as assisted living, home care, hospitals, long-term care, and rehabilitation facilities.
Consider the model used for the interdisciplinary geriatric care teams at your current practicum site. Compare this model to models used at other sites.
Reflect on how the role of the advanced practice nurse differs according to the site of care.
Select one of the three case studies. Consider how care should be facilitated for the patient in the case you selected based on the model used for the interdisciplinary geriatric care teams at your practicum site.

Statistical Thinking in Health Care

Read the following case study.

Ben Davis had just completed an intensive course in Statistical Thinking for Business Improvement, which was offered to all employees of a large health maintenance organization. There was no time to celebrate, however, because he was already under a lot of pressure. Ben works as a pharmacist’s assistant in the HMO’s pharmacy, and his manager, Juan de Pacotilla, was about to be fired. Juan’s dismissal appeared to be imminent due to numerous complaints and even a few lawsuits over inaccurate prescriptions. Juan now was asking Ben for his assistance in trying to resolve the problem, preferably yesterday!

“Ben, I really need your help! If I can’t show some major improvement or at least a solid plan by next month, I’m history.”

“I’ll be glad to help, Juan, but what can I do? I’m just a pharmacist’s assistant.”

“I don’t care what your job title is; I think you’re just the person who can get this done. I realize I’ve been too far removed from day-to-day operations in the pharmacy, but you work there every day. You’re in a much better position to find out how to fix the problem. Just tell me what to do, and I’ll do it.”

“But what about the statistical consultant you hired to analyze the data on inaccurate prescriptions?”

“Ben, to be honest, I’m really disappointed with that guy. He has spent two weeks trying to come up with a new modeling approach to predict weekly inaccurate prescriptions. I tried to explain to him that I don’t want to predict the mistakes, I want to eliminate them! I don’t think I got through, however, because he said we need a month of additional data to verify the model, and then he can apply a new method he just read about in a journal to identify ‘change points in the time series,’ whatever that means. But get this, he will only identify the change points and send me a list; he says it’s my job to figure out what they mean and how to respond. I don’t know much about statistics — the only thing I remember from my course in college is that it was the worst course I ever took– but I’m becoming convinced that it actually doesn’t have much to offer in solving real problems. You’ve just gone through this statistical thinking course, though, so maybe you can see something I can’t. To me, statistical thinking sounds like an oxymoron. I realize it’s a long shot, but I was hoping you could use this as the project you need to officially complete the course.”

“I see your point, Juan. I felt the same way, too. This course was interesting, though, because it didn’t focus on crunching numbers. I have some ideas about how we can approach making improvements in prescription accuracy, and I think this would be a great project. We may not be able to solve it ourselves, however. As you know, there is a lot of finger-pointing going on; the pharmacists blame sloppy handwriting and incomplete instructions from doctors for the problem; doctors blame pharmacy assistants like me who actually do most of the computer entry of the prescriptions, claiming that we are incompetent; and the assistants tend to blame the pharmacists for assuming too much about our knowledge of medical terminology, brand names, known drug interactions, and so on.”

Post an explanation of strategies you would use to minimize or address barriers, including resistance to change, related to your strategic plan.
Discussion: Team Building and Addressing Barriers to Planned Change

Planning and implementing change can be quite challenging, especially in a complex health care environment. No matter what kind of change is undertaken, there are likely to be some obstacles. Yet, wise leader-managers, and those with whom they work, recognize that change is necessary and revitalizing; they see that planned change can not only contribute to organizational sustainability but also promote high-quality care and positively impact patient outcomes. These leader-managers harness a larger vision and engage in forethought and analysis to minimize and address barriers to planned change.

Based on what you have learned about the change process and strategic planning, what barriers do you think may arise related to the change you are proposing through your Course Project? How would you cultivate stakeholder involvement and promote team building for your planned change?

As you think about these questions, consider the following statement: “The system will evolve based on changes in the behavior of the people in the system” (Nesse, Kutcher, Wood, & Rummans, 2010, p. 27).

Do you agree? Why or why not? How does this relate to the specifics of your strategic plan?

To prepare:

Review the information presented in the Learning Resources, including Dr. Carol Huston’s comments on addressing barriers to implementing a strategic plan change. Consider insights related to change theory, common barriers to change, and strategies for addressing these barriers that relate to the implementation of your proposed change.

Identify specific barriers you are likely to encounter with your proposed change, including resistance to change. Evaluate strategies that may be used to minimize or address these barriers.

Think about the value of stakeholder involvement in planned change, as well as the specifics of stakeholder participation in your strategic plan to champion the change. Who should be involved in strategic planning and at what point? How could they serve as change champions?

Post an explanation of strategies you would use to minimize or address barriers, including resistance to change, related to your strategic plan. Explain how you would promote stakeholder involvement for your proposed change, and encourage them to become change champions.

Read a selection of your colleagues’ responses.

Respond to at least two of your colleagues on two different days using one or more of the following approaches:

Provide feedback on their strategies for addressing barriers to change.

Suggest additional strategies for addressing barriers or resistance.

Offer additional insights for identifying appropriate stakeholders.

Suggest additional stakeholders or change champions to be involved.

Describe the provisions for involvement of family and nominated carers in mental health treatment within the NSW Mental Health Act 2007 .
Assessment 1: Written Assignment – Essay Weighting: 40% 2,500 words

DUE SUNDAY AUGUST 16, 2015 midnight (WEEK 3)

Assignment guide:

The historical care and treatment of people with a mental illness has provided strong evidence to underpin the need for less paternalistic, more inclusive models of care. The recovery movement has driven the necessary revisioning of mental health care service delivery on a worldwide scale. The empowerment and inclusion of consumers and carers in care planning and treatment is the foundation of the recovery movement. This shift has been reflected at all levels of service planning and care delivery and also within the legislation that governs the care, treatment and control of people who are mentally ill or disordered (NSW Mental Health Act 2007).

Assignment questions:

This assignment is organised within 4 parts:

Part a) requires you to describe the provisions for involvement of family and nominated carers in mental health treatment within the NSW Mental Health Act 2007 .

Part b) requires you to review a variety of literature and discuss the meaning of recovery within the context of mental health.

Part c) requires you to discuss nurses’ inclusion of consumers in planning their mental health care and treatment and how this participation has changed with the advent of the recovery movement.

In part d) describe how the active participation of consumers and nominated carers in planning care and treatment contributes to a person’s recovery in a mental health context.

Finally, write a short conclusion that analyses and synthesises your discussion of the above areas with the implications for nurses across all areas of nursing practice.

TIPS FOR WRITING THIS WORK:
• This is an essay and therefore there should not be any headings within your work. Each section of discussion should end with a sentence or two that lead into the next section.
• Construct a short introduction that informs the reader of the areas you will be discussing in your essay (the four sections).
• Note that 50% of the marks for this work are attributed to how well you argue (critique) the information you are presenting, how well you write it and how you reference. Please apply a consistent approach to referencing (needs to be school style).
• As a guide, if you find one reference for each area of discussion you can only describe what the authors have said and this will attract a pass mark at best. To critically analyse, you need to source a few references for each point you are making and compare and contrast the authors different findings, or point out that they all agree (reach a conclusion).

How do you think the political climate has affected the choice of policy tools and the behavioral assumptions by policymakers?

How do you think the political climate has affected the choice of policy tools and the behavioral assumptions by policymakers?

1) Edith Elizabeth Gonzalez
The state health policy in America is the Patient protection and affordable healthcare policy which came into being as a result of the then president Barak Obama enacting a law to allow all employees to be contributors to a scheme which insures their health. It is famously known as the Obama care. The policy came into being in 2010 and saw all the employers enlist their workers into health insurance schemes. In this sense every American citizen are entitled to a health insurance cover which they contribute in part. The insurances policy utilized the tools like the cheat sheets the work plan the self-audit tool and the and lastly exchange function checklist tool to implement the policy (Hall & Lord 2014).
How do you think the political climate has affected the choice of policy tools and the behavioral assumptions by policymakers?
Yes! The political climate did affect the implementation of the policy by deflecting the choices of the tool used in the implementation of the policy. The policy makers opposed to the policy influenced the choice of the tools that would allow the government to be directly involved in the implementation of the plan and also caused the oppositions behave in the manner likely to suggest they were opposing the whole plan. Voting nearly pulled it down. The current president used the scheme as a campaign toll stating that it is making Americans poorer. Politics also burred the financial allocation for the program and influenced the structure of the plan in total (Tanenbaum 2016). How have professional nursing organizations been involved in this policy issue? The healthcare professional have been involved by being mandated to encourage the customers register in the plan and start contributing. They have been asked to advice the employees to ensure their employers enroll then and contribute towards the scheme.
Develop a few talking points to inform other health care professionals regarding this issue. The healthcare practitioners are mandated with the quality wellbeing of the citizens of the community and therefore need to be consulted in the policy formulation testing adjustment and implementation levels so that the ultimate goal of the policy is achieved amicably. They are in close contact with the patients and need to be involved in educating the customers on the importance of the health insurance cover and encourage them to be contributors and so benefactors of the policy.

2) Dayami
Identify a state health policy and the tools used to implement the policy.
Health policyof a state is government initiative in collaboration with other concerned policy makers that enable them to attain a present and future objective of high quality and affordable health care services and system that work for the people. This is outlined and articulate in the processes and formulations that are guided by the legislation opinions guidelines regulations and set standards. In America the current healthcare policy is the Patient Protection and Affordable health care that brought in Obama care. It was enacted in 2010. It required the employers to provide their workers with health care insurances. This necessitated every citizen to have health insurance and contribute towards it. The tools used to implement the policy include the cheat sheets and work plan the self-audit insurance tool and the checklist for the exchange function tool (Hall & Lord 2014).
How do you think the political climate has affected the choice of policy tools and the behavioral assumptions by policymakers?
The political climate deflected the choice and use of the various tools available by opposing direct government involvement in the structuring and financing of the implementation the policy thus inducing the use of the above tools (Tanenbaum 2016).
How have professional nursing organizations been involved in this policy issue?
The professional nursing has been mandated to ensure the policy is being followed by both employees and employers as they are encouraging them to be part of the policy implantation team. They have been asked to initiate bodies that will link the employees to employers that will advocate for the benefits of the program.
Develop a few talking points to inform other health care professionals regarding this issue.
The healthcare professionals need to take part in the policy implementation so as to ensure attendance of the affordable quality healthcare services to the Americans.
Encourage all citizens to gladly contribute to the scheme and provide all the necessary information regarding the policy.
Employ all available mechanism to ensure the objectives of the policy is achieved.
